PROCEDURES AND RELATED INFORMATION 1.
Application forms are available at the offices of DE Phones/Telegraph/Customer Service Centers and PTCL web site.

Application duly filled up be deposited at any Customer Service Center. Incomplete application may delay the process.

Separate applications are required for different addresses.
