WEEKLY PROGRESS REPORT ARCELLANA, Anthony A. Week #: 1 January 8 – 12, 2007 January 8, 2007  I was able to disassemble, clean, repair and assemble back a defective POS printer, accomplish Job Order (JO) Form and be able to output it as good  I “witnessed” the delivered parts as a logistics procedure and to make sure that received parts are correct and their quantities are exact  I was able to make delivery evaluation to ensure that the COO’s (Country of Origin), part numbers, package numbers and if the parts / equipments received are correct and to indicate if it is hazardous or not January 9, 2007  I was able to receive POS Printer Model 1 and signed the RMER (Return Material and Equipment Report) as a proof that I receive it  I was able to accomplish JO Form and log the printer to our POS log sheet so that we have records of printers that come in and come out in our section January 10, 2007  I was able to determine the defect of the previously received printer. The found defect of the printer was skip printing  I was able to fix the printer by adjusting the journal and test the printer again to verify if the printer is now in good working condition  I was able to clean and burn-in the printer after I was finished repairing it  I was able to complete the JO Form and output it to the warehouse January 11, 2007  No pending defective printer since we already fixed and outputted all the pending printers  I did not receive any defective printer  I went to other section of the Repair Center (Monitor and OMRON) to check out what they do and if i could help them in repairing and burning out their pending January 12, 2007  I was able to receive POS Monitor, accomplish JO Form and logged it to our log sheet  I was able to test and check what defects the printer have Noted by: ______________________ Ms. Marissa Bernal

Week #: 2 January 15 – 19, 2007 January 15, 2007  I was able to fix, test and clean a POS Monitor  I “witnessed” the delivered parts as a logistics procedure and to make sure that received parts are correct and their quantities are exact  I was able to make delivery evaluation to ensure that the COO’s (Country of Origin), part numbers, package numbers and if the parts / equipments received are correct and to indicate if it is hazardous or not January 16, 2007  I was able to accomplish and complete JO Form, log it in our log sheet and output the monitor as good in the warehouse  I was able to receive defective POS Model 4 Printers which is subjected for repair, test and burn-in January 17, 2007  I was absent because I have fever and not feeling well January 18, 2007  I was able to test and determine the defect of the printer I received previously  I was able to repair and clean the printer  After repairing, I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working January 19, 2007  I still have pending printers that is subjected for repair, test and burn-in and continued testing and determining the defect of the printer I received previously  I was able to repair and clean the printer  After repairing, I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working

WEEKLY PROGRESS REPORT ARCELLANA, Anthony A. Week #: 3 January 22 – 26, 2007 January 22, 2007  I was able to accomplish forms and do the necessary procedures in requesting new parts as a replacement for the defective parts of the received printers  I was able to receive the newly issued parts of the defective POS Printer Model 4  I was not able to repair the POS printer since it has so many broken parts and some of these are irreplaceable since we didn’t have replacement parts for these  I was able to accomplish and do the procedures for scrapping of the defective POS printer and parts January 23, 2007  I was able to receive defective POS printers which is subjected for repair and test  I was able to accomplish JO Forms and do necessary procedures in receiving defective printers such as signing the RMER and logging it to our log sheet January 24, 2007  I was able to test and determine the defect of the printer I received previously  I was able to repair and clean the printer  After repairing, I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working January 25, 2007  I still have pending printers that is subjected for repair, test and burn-in and continued testing and determining the defect of the printer I received previously  I was able to repair and clean the printer  After repairing, I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working January 26, 2007  I updated our log sheets by writing the invoice numbers of the recently outputted printers and accomplish other pertinent documents for proper filing and documentation of the activities we did in the POS section of the Repair Center  I went to Reutilization section of IBM Parts Center to look for scrapped printers that we can repair  I was able to pull out POS Model 1 Printer and tried to repair it Noted by: ______________________ Ms. Marissa Bernal

Trainee Supervisor WEEKLY PROGRESS REPORT ARCELLANA, Anthony A. Week #: 4 January 29 – February 2, 2007 January 29, 2007  I continued fixing the POS Model 1 Printer that I pulled out from the Reut Section  I adjusted some parts of the printer such as the Ribbon Guide and did the necessary things to fix the printer  I tested and evaluated the printer if I can output it as good  At the end of the day, I’m still not satisfied with the performance of the printer after I adjusted, fixed and tested it  I decided to put it back to Reut and declare it as scrap January 30, 2007  No pending defective printer since we already fixed and outputted all the pending printers  I did not receive any defective printer  I went to other section of the Repair Center (OMRON Section which is responsible in repairing ATM Machine parts such as cassette, pick and feed, and issue unit) and they taught me how to test and burn-in cassette (cassette is an ATM part in which the money to be issued is stored)  I was able to test and burn-in cassettes that they already fixed and evaluate the performance of the cassette that was subjected for testing and burning-in January 31, 2007  I went to the Monitor Section of the Repair Center and they taught me how to repair, test and burn-in LCD Monitors  I was able to repair LCD Monitor and subjected it for burn-in and testing  I was able to receive a defective POS Model 1 Printer which is subjected for repair, test and burn-in February 1, 2007  I was able to test and determine the defect of the printer I received previously  I was able to repair and clean the printer  I was able to burn-in the printer to check if its really good for output  After repairing, I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working February 2, 2007  No pending defective printer since we already fixed and outputted all the pending printers  I did not receive any defective printer

 I went to NDF (No Defect Found) Section of the Repair Center and be able to format a working hard disk  I updated our log sheets by writing the invoice numbers of the recently outputted printers and accomplish other pertinent documents for proper filing and documentation of the activities we did in the POS section of the Repair Center

Week #: 6 February 12 – February 16, 2007 February 12, 2007  I “witnessed” the delivered parts as a logistics procedure and to make sure that received parts are correct and their quantities are exact  I was able to test and determine the defect of the printer then I repaired and cleaned the printer, and finally I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working  I was able to make delivery evaluation to ensure that the COO’s (Country of Origin), part numbers, package numbers and if the parts / equipments received are correct and to indicate if it is hazardous or not February 13, 2007  I was able to receive defective POS printers which is subjected for repair and test  I was able to accomplish JO Forms and do necessary procedures in receiving defective printers such as signing the RMER and logging it to our log sheet  I was able to test the defective printer and determine the defect February 14, 2007  I repaired the previously received printers, output it to the warehouse and that contributes to the savings of the company  I was able to receive new defective printers and tested it to determine its defect February 15, 2007  I was able to repair the previously received printer and do the necessary procedures to output it as good and that contributes to the savings of the company February 16, 2007  I still have pending printers to repair so I continued repairing and do the necessary things to output the printer as good and that contributes to the savings of the company

Week #: 7 February 19 – February 23, 2007 February 19, 2007  I “witnessed” the delivered parts as a logistics procedure and to make sure that received parts are correct and their quantities are exact  I was able to test and determine the defect of the printer I received previously then I repaired and cleaned the printer, and finally I accomplished and completed the printer’s JO Form and output it to the warehouse as good and working  I was able to make delivery evaluation to ensure that the COO’s (Country of Origin), part numbers, package numbers and if the parts / equipments received are correct and to indicate if it is hazardous or not February 20, 2007  I was able to sort the used and stock parts of the POS section then after sorting I scrapped the unusable parts then I organize the shelves to make it easier for us to see for the part numbers and search for parts. February 21, 2007  I taught my co-trainee how to repair a model 4 POS printer and guide him with the proper steps and procedures to be able to output the said printer as good.  I was able to receive new defective printers and tested it to determine its defect February 22, 2007  I was able to repair the previously received printer and do the necessary procedures to output it as good and that contributes to the savings of the company February 23, 2007  I accomplish forms to be able to scrap the unusable model 4 POS printer assembly covers  I updated our log sheets by writing the invoice numbers of the recently outputted printers and accomplish other pertinent documents for proper filing and documentation of the activities we did in the POS section of the Repair Center
