Occupational Therapy Diagnosis and Assessment

Rem has difficulty in Job performance specifically in encoding of utility bills using forms secondary to problem in mental function and awareness due to Autism Spectrum Disorder

II.

Intervention Plan

Frame of Reference The Behavioral frame of reference will be used to ensure that positive associations can be made with the activities he dislikes, and to establish more adaptive behavioral responses when performing these activities. Choosing and grading activities according to his skill level while simultaneously using different behavioral modification techniques to facilitate interest and performance may establish the skills necessary to be able to participate better in the following occupations. Acquisitional FOR will be used to improve the teaching and learning process in order to achieve specific skill and appropriate behavior required for optimal performance necessary for the needs of the client. The FOR emphasizes on the context of environment, functional behaviours and learned skills that may improves the client’s functional ability in the areas of concern.

LTG1: To perform job performance encoding of utility bills using forms independently within 6 months of OT sessions. STG1: Rem will be able to sort utility bills according to type (water/ electrical) given minimal verbal prompts and cues within 3 OT sessions. STG2: Rem will be able to arrange utility bills in order according to date (from oldest date to the most recent date) given minimal verbal prompts and cues within 3 OT sessions. STG3: Rem will be able to search the due dates of utility bills given minimal verbal cues within 3 OT sessions. STG4: Rem will be able to write the utility bill amount and dates in the correct columns of the form given minimal cues in 3 OT sessions. STG5: Rem will be able to check his own work by matching the amount and date written in the column if correct given minimal verbal prompts in 7 OT sessions. Therapeutic use of self: Active friendliness will be used to encourage the client’s participation and motivation and strengthen the rapport of the client shifting into passive friendliness to promote client’s initiation to socially interact with the therapist. Environmental modification technique: Placing him in an environment that has less distractions for him to be able to attend to and finish the activities being given. Also, highlighting the prices and dates of the bill may be utilized for him to be able to easily scan the document for him to efficiently arrange.

Behavioral modification technique: Positive reinforcement such as high five and “good job” may be used when the client follows instructions, attends and finishes his tasks. Prompts and cues may also be given during activities to help the client when he is having difficulty performing a task. Therapeutic Use of Activity : Preparatory Activities:   

Carrying of heavy weights while transferring containers Pushing of heavy weights while completing puzzles Obstacle course with 3 – 4 steps

Sorting of electric and water bills according to dates (months, day, year ) Sort the water and electricity bills Sorting of amount of utility bills

For arranging in order   

Arranging of months, days and years in order using flash cards Arranging of electrical and water bills according to dates ( months, day, year) Arranging of electrical and water bills from lowest to highest

For searching  

Searching for due date by months, day, year Searching for the amounts of utility bills
