The advantage of DBS is leaving the patient and his brain intact and available for any future therapies that may come up. The morbidity of the stimulation procedure is also less than lesional surgeries. DBS leads are programmed with the help of an external programmer. This gives the surgeon flexibility in optimizing the current and electrode contacts in each individual patient to suit his requirements. The alternative treatment is Stem cell treatment or Stem cell treatment India. Parkinson's Disease is a degenrative disease caused by loss of dopamine-producing cells in the substantia nigra. So regenerative or stem cells have to be isolated from brain, bone marrow, skin, fat, skeletal muscle and other visceral organs. Adult stem cells appear to be the ideal candidates to serve as the cellular reservoir. They will optimally be transplanted autologously, decreasing to a minimum the risk of immune rejection and, because of their more restricted differentiation potential, are considered less prone to form tumors in the host. Thalamotomy has very limited role in the present scenario for PD surgery. It is only useful in relieving Parkinsonian tremor. Thalamotomy is not effective in alleviating other symptoms, like rigidity, bradykinesia and postural instability seen in PD patients. For the treatment of these symptoms we need to use other surgical targets. When undertaking thalamotomy or thalamic stimulation, it is important to realize this and educate the patient and physician about the need for continuous medical management of other Parkinsonian symptoms following surgery. Pallidal stimulation : Pallidotomy has been found to be most effective in controlling drug induced dyskinesias, dystonia and other associated with off phase symptoms. The surgical target for pallidotomy is the most vetro-medial part of the globus pallidum known as Globus pallidum internus (Gpi). This part of the Gpi is located in close relationship to the internal capsule and optic tract. Internal capsule carries nerve fibers that are responsible for motor function and any damage to these fibers can cause weakness on the opposite side of the body whereas the optic tract carries the fibers for the vision and any damage to these fibers can cause visual field defect. These side effects can be avoided by using the expertise and experience of a functional neurosurgeon.
