PERFORMANCE MONITORING REQUIREMENTS
6.1
IDENTIFICATION OF PERFORMANCE INDICATORS As a minimum, the following shall be measured / monitored onboard our facilities: • Customer satisfaction - are we meeting customer expectations and requirements?

It is the responsibility of the Designated Person Ashore (DPA) / Process Owner, in consultation with the Offshore Installation Manager (OIM) and Discipline Superintendents, to identify the performance indicators required to assess the functioning of the CAMS at facility level. This shall include significant risks and associated facility objectives and targets.

The key monitoring and measurement criteria identified shall be documented in each facility’s Performance Monitoring / Measurement Plan and may be further documented in facility FOPs. The frequencies and responsible parties for completing the monitoring and measurement activities shall also be specified within the facility’s Performance Monitoring / Measurement Plan. The results of monitoring and measurement activities shall be reviewed for conformance to specific requirements at various facility Meetings, at which stage provisions shall be made for corrective action and a Non Conformance Report may be opened, dependent on the severity of the non-conformance.

Results of the monitoring and measurement activities provide valuable information that must be used to improve the functioning of the CAMS, thereby making these activities an important continuous improvement function. Conformance to monitoring and measurement requirements is essential and shall be assessed through the internal audit process. The objectives of the monitoring program may vary depending on the particular activity, its location and the particular issues associated with it. Monitoring should include: • Checks on the effectiveness of design and operation, • Compliance with statutory requirements / standards, • Detection of unpredicted environmental changes, and • Measurements of recovery. Comprehensive records of each of these parameters shall be maintained on each facility and the information shall reviewed by facility management to provide a rational basis for evaluating facility performance and any improvements required.

Note: Baseline or initial surveys are a prerequisite for field environmental monitoring programs to allow evaluation of results against pre-activity conditions. The monitoring objectives and requirements should be defined during the environmental screening and assessment process and the monitoring program tailored to meet these objectives. As a minimum, the following parameters should be monitored at all operational locations:

Aqueous discharges - volume and composition of all discharges,

Atmospheric emissions - fuel consumption, estimated emissions of CO2, SO2, NO2, hydrocarbons and CFCs,

Accidental releases - location, time, type and quantity of release.
INTERNAL AUDIT REQUIREMENTS

Systematic internal auditing of the CAMS shall be conducted at planned intervals in order to evaluate:

• The compliance of the CAMS to the requirements of ISO 9001, ISO 14001, OHSAS 18001, and the ISM Code 2010; and

• The effective implementation and maintenance of the CAMS in line with Company requirements, policies and objectives.

Internal audits shall be planned on the basis of risk, and the importance of the processes and scope of activities. The outcome of Management Reviews, previous audits, customer complaints and reported non conformances shall also be assessed when determining audit frequency.

An Internal Audit Schedule shall be prepared by the DPA/Process Owner in consultation with the relevant Operations Manager (Specific Unit) and OIM for each facility. These audit schedules shall be finalized at the beginning of each calendar year, as part of each facility’s key performance indicators (KPIs). Once the schedule has been finalized, the necessary arrangements shall be made to perform the audits. Internal audits shall be conducted at least once pear year (comprehensive CAMS audit) or at least quarterly (limited scope audit). Independent facility-based 3rd Party Audits may also be performed. Onshore office-based audits shall be conducted at least once per year. The frequency of internal audits (facility and office) may be increased as deemed necessary by the DPA/Process Owner to verify conformance with Company requirements, or when found necessary at an CAMS Review Meeting or in response to an identified undesirable occurrence or adverse trend. 6.5
INTERNAL AUDITORS To be able to assess and report non-conformances within any discipline or department and to suggest positive corrective action, internal auditors shall be selected based on:

• Sufficient qualifications and experience in conducting audits; and • Independence of the activity being audited (to ensure objectivity and impartiality). 6.6
AUDIT PLANNING, PREPARATION AND EXECUTION Internal audits shall be planned, prepared and executed by the appointed Audit Team in accordance with the requirements of the Internal Audit Procedure. The Lead Auditor shall manage the audit process. Contracted / external auditors may be utilized and shall be equally bound by the requirements of this procedure. Internal audits shall cover AMO operations activities (as managed by AMO) as well as those of major sub-contractors to: • Identify weaknesses in QHSE management programs / activities;

• Improve risk management (reduce HSE risks) and reduce operating costs;

• Ensure compliance with standards, legislation and client / contract requirements;

• Ensure compliance with CAMS requirements and effective document control; • Verify predictions from facility-based risk assessment; • Ensure compliance with competency profiles;

• Ensure achievement of personnel training objectives; • Evaluate monitoring and follow-up programs; and • Assess emergency response plans and preparedness.

Office-based audits shall be conducted by the DPA / Process Owner, or by suitably qualified and experienced personnel appointed by the DPA / Process Owner. All areas of the business pertaining to the operation of the Company vessels shall be audited. Note: Unless impracticable, auditors shall not be permitted to carry out audits in areas of work where they have direct responsibilities.
The Client shall be notified of all scheduled audits and will be afforded the opportunity to participate in audits if desired. Objective findings shall be brought to the attention of the Auditee in the activity being audited and shall be acknowledged by the Auditee. Internal auditors shall provide documented evidence to substantiate their findings, specifically where findings relate to significant risks. The results of the audit shall be reported on Audit Reports. All non-conformances identified during internal audits shall be rated as either a: • Non conformance (high); • Observation (medium); or • Recommendation (low).

On conclusion of the audit, the Lead Auditor (facility-based audit) / DPA / Process Owner (office-based audit) shall hold a Closing Meeting to present a summary of the audit findings. 6.7
AUDIT REPORTING AND CLOSE-OUT Audit Reports shall be prepared by the Lead Auditor and submitted to the DPA / Process Owner. The DPA / Process Owner is responsible for office-based internal audit reports and is also responsible for maintaining records of all internal audits and for tracking non conformances and corrective actions. Responsible persons shall ensure that corrective actions are performed timeously to eliminate detected non conformities and their causes (refer to the Corrective and Preventative Action Procedure).
