This is to certify that Mrs.Orawan Somboune born in 1963 residing at 8/31 Burapa 7, Moo 5 Srikan, , Don Mueang District Province, Bangkok, Thailand has requested the District Office to issue a letter of affirming that she has given maintenance to Onvanya Phuttharak her daughter who was born of unmarried husband. Mr. Chusak Phuttharak who was born August 10, 1965 has not registered a child legitimating at the Office of Don Mueang District, and that since separation, Onvanya Phuttharak has solely been under the father’s/mother’s maintenance.

After examining the witness and verifying the document, the District Office has been assured that Mr. Chusak Phuttharak is the sole person giving maintenance to his daughter namely Orvanya Phuttharak pursuant to Section 1566 (2) of the Civil and Commercial Code Book V.
