Boost Mobile Landlord Consent to Install Boost Mobile Illuminated Sign Address of Location: _______________ _______________ _______________ Boost Mobile (“Boost”) has authorized __________ [insert name of Dealer] (“Dealer”), through its Distributor _______ [insert name of Distributor], to install a Boost Mobile Illuminated Sign (“Signage”) with Boost’s logo on Dealer’s storefront. Because Dealer leases its store location at [insert address] from ________ (“Landlord”), Boost requires Landlord’s acknowledgement of its consent for the sign to be installed, and that the sign will remain the property of Boost. Landlord agrees the Signage may be installed at the address above and acknowledges the Signage remains the property of Boost and reserves the right to remove, revise, or reposition the Signage at any time. Landlord acknowledges that Dealer’s lease has an expiration date of _________. Landlord agrees to notify Boost Mobile at [email protected] or at phone number #949-789-4722 at least fifteen business days prior to any eviction of Dealer or termination of Dealer’s lease so Boost can make arrangements to retrieve the Signage. Boost will handle the relocation or removal in a timely manner.
