Ensure the worksite is free from Flammable / Toxic vapor.
Non – compliance of KOC F & S Regulations and HSE MS Procedures
Refer Appendix – 10 of KOC F& S Regulations, HSEMS Procedures - Doc. No. KOC.SA. 004 “Permit to Work”; Doc. No. KOC.SA. 022 “Electrical Multilock & Tagout”.
Electrically Energized Panel
Asses the risk & isolate the power supply to the panel.
Unintended Shutdown of Facility / Equipment
Evaluate the risk & identify the safety override (such as putting CO2 Flooding System in manual mode) required for the work.
Unauthorized Work
Take a Cold Work Permit and other associated permit from Asset Owner.
Improper Fuse
Ensure the availability of spare fuse (of proper specification & rating) as required for replacement.
Ensure to put the CO2 Flooding System in manual mode in case the panel is located inside a substation.
Record & authorize CO2 Flooding System bypass as per HSEMS Procedure “Plant Safety Override Registration” Doc. No. KOC.SA.020.
Evacuate the substation safely in case of fire alarm.
Ensure the person (who will replace fuse) is authorized as per procedure for the relevant voltage rating.
Ensure the isolation of electrical power before replacing the fuse.
Use insulated hand gloves & safety shoe.
Blown Up Fuse
Ensure the replaced fuse has the same rating & specification as the original one.
Improper Closure of Panel
Ensure the panel is properly & safely closed before restring the power supply.
Restoration of Power Supply
Take the approval from Permit Applicant & Permit Issuer prior to energizing the panel after fuse replacement.
Ensure proper wind up & housekeeping at worksite.
Normalize safety override after evaluating the risk.
Close the permit & associated permits / certificates.
Accidental Release of CO2 Flooding System inside Substation.
Unauthorized Personnel Fuse Replacement Electric Shock
