Instructor Feedback Form

HSU, Fall 08

Date: Student’s Name:______________________ Assignment:___________________ Student: be sure you have this sheet, the assignment sheet, and any materials you are working with when you speak to me, someone in the writing center, or someone in the writing lab. Instructor or Tutor: Please sign here only after you have worked with the student. Thanks! If you have any questions or constructive feedback about the form, please email me at [email protected]. Please help facilitate this process by reading the entire sheet and focusing on giving verbal, not written, feedback on all but mechanical/local concerns.

Printed Name: _____________________________ Signature __________________________ Student: Be sure that you write down your three primary concerns. They may be QUESTIONS, such has “What does he mean by analysis?” or concerns like “I don’t know if there’s enough evidence here. What do you think?” In order for the instructor or tutor to give you good feedback, they need to read for themselves what the assignment is—thus, have the assignment sheet with you.. Process: There is room for you to write down three concerns/questions. Write them down before you meet with the instructor or tutor. Next, when you meet with them, tell them what your three concerns questions are. Then let them read your paper or you may read it to them. Finally, they will give you VERBAL feedback for each question. Before the discussion ends, the student should WRITE DOWN on this form what they understand and make sure that they clearly understand what the instructor or tutor said about each question or concern. It is vital that the student seek and the instructor or tutor give confirmation that the message/feedback was understood clearly. This way, when the student revises at a later time, they have a clear record and document of what was said.
