SK™ External Skeletal Fixation System
The SK™ External Skeletal Fixation System The History of SK™ ESF Technology The SK™ ESF System is designed to simplify the application and management of external skeletal fixators. The SK™ ESF clamp supports use of positive thread pins directly through the primary clamp bolt, allows the surgeon to easily add a fixation clamp after the fact as needed, and provides the freedom to utilize a great variety of fixation pin diameters and styles. In addition, the SK™ ESF System supports sleeved pre-drilling and targeting of fixation pins. Traditional KE frames and new KE-like frames typically depend on complex frame geometries, connecting rod augmentation, and/or multiple full-pins to achieve adequate frame strength. In contrast, the SK™ ESF System features dramatically improved connecting rods that make use of simpler frames a clinical reality. The SK™ ESF System dramatically changed standard concepts of veterinary ESF by placing less emphasis on full-pins and frame complexity while promoting biological technique and safe pin corridor philosophy to reduce patient morbidity. By-products of simple frame constructs include: economy of hardware, reduced operative time, and less required instrumentation. The SK™ ESF System is available in three sizes; mini, small, and large.
Review of SK™ ESF Clamp Design The user-friendly design of the SK™ single clamp allows for pre-drilling as well as placement of positive thread pins through the pin-gripping clamp bolt. This eliminates the difficulty historically associated with the use of positive profile threaded pins. The gripping mechanism of the SK™ ESF clamp allows a variety of different pin sizes and types to be utilized at the surgeon’s discretion. The split nature of the SK™ clamp allows quick and easy addition or removal of a clamp without requiring frame disassembly. Simple wrenches are the only required instrumentation; however, optional drill sleeves are available to simplify pre-drilling. The aluminum clamp body is comprised of two different parts (B-1 and B-2). Each has a pair of holes, the left one to receive the primary pin-gripping bolt (pb) and the right one to receive the secondary bolt (sb). None of these holes have threads except for the secondary bolt hole in B-1 body part. After each use, a clamp may be disassembled for proper cleaning. The B-1 and B-2 body parts must not be interchanged when the clamp is reassembled for another use, or it will not work properly. Proper assembly of the clamp is shown in the photo at right. The large rod-gripping channel (R) is in the center of the clamp.The primary pin-gripping bolt (pb) has a washer with a slot or meniscus (arrow) which enables a wide range of pin diameters to be securely gripped by the clamp. The clamp is tightened by the secondary bolt and by a nut (n) on the primary pin-gripping bolt. Alternative tightening of each bolt will maximize clamp performance. The secondary bolt should be viewed as a second pair of hands to be utilized to position and hold clamps in place while aiming drill bits, drilling, and placing pins. When one becomes comfortable utilizing the secondary bolt as an assistant it becomes very easy to reduce fractures and apply frames. The secondary bolt converts an empty clamp into both a drill guide and a targeting device. While the mechanical properties of the SK™ ESF System allow minimal use of full-pins, they can be easily targeted without additional expensive instrumentation.

The SK™ ESF Advantage Review of SK™ ESF Mechanics The most powerful way to simplify the ESF method lies in mechanical frame improvements that are not dependent on full-pins or complex frame geometries. In this respect, the SK™ ESF System is designed to be dramatically different from the KE and new KE-like systems that utilize traditional 1/8” and 3/16” connecting rods. Mechanical testing of KE frames reveals the connecting rod as the “weak link” of simple fixator frames. In retrospect, the choice of KE rods utilized was and still is poorly matched to veterinary pin diameters and patient demands; dictating that veterinary surgeons utilize complex ESF frames. By eliminating the weak link of simple frames, the SK™ ESF System provides a simple but powerful alternative to complex frame geometry.

Comparative Bending Strength of ESF Rods
Simple frames, the ability of clamps to effectively grip a wide range of different pin diameters, and the need for minimal instrumentation make the SK™ ESF System the most economical ESF choice. Reduced use of full-pins minimizes pin tracts and related morbidity while simplifying staged disassembly and post-operative care. Most importantly, use of half-pins instead of full-pins encourages and facilitates optimal pin centering and use of safe pin corridors.

Isolated connecting rod testing does not tell the entire story. Actual testing of ESF frame models confirms that frame simplification can be accomplished by incorporating strong connecting rods. Typical ESF frames were constructed of large SK™ components with medium pins as well as medium KE components with medium pins. By comparing frames with equivalent geometries and identical pins, the actual impact of the connecting rod on mechanical performance can be seen (Bronson, D.G., Ross, J.D., Welch, R.D., Proceedings of Veterinary Orthopaedic Society Annual Meeting, 1999). Recently published mechanical testing of currently available veterinary ESF devices confirms the ability of larger diameter connecting rods to allow clinical use of simpler frame geometries (White, D.T., Bronson, D.G., Welch, R.D.: A Mechanical Comparison of Veterinary External Skeletal Fixation Systems. Veterinary Surgery 32:507-514, 2003). The goal of the SK™ ESF System is not to generate ever-increasing frame stiffness, but to achieve clinically appropriate strength while using simple, patient friendly frames.
1 Axial Compression The large SK™ Type l-a frame exceeds the strength of the KE Type l-a, KE Type l-b and the modified Type II KE (one full-pin per fragment and the balance half-pins). The large SK™ Type I-b provides 60% of the axial strength and stiffness of the most complex KE frame (Type lll).
2 Cranio-Caudal Bending The large SK™ Type l-a frame exceeds all KE frames except the Type lll.
3 Medio-Lateral Bending Comparable Type l-a and l-b frames have an approximate fourfold increase in strength when constructed utilizing the large SK™ rods.

SK™ ESF Q & A What pin sizes does each SK™ ESF clamp support? The mini SK™ clamp is designed to grip pin shank diameters of 0.035” (0.9mm) to 2.5mm. With current positive thread pins, the largest pin used is typically the 2.0mm shank INTERFACE™ or CENTERFACE™ pin which features a 2.5mm thread diameter. Miniature INTERFACE™ pins of 0.078” and 0.062” diameters are also popular choices. While the mini SK™ clamp can hold pins as small as 0.035” and is very compact, there will be kittens, toy breeds, and exotics with bones so small they will be unable to wear mechanical frames. Such patients will benefit from acrylic ESF frames constructed using miniature INTERFACE™ pins. The small SK™ clamp is designed to utilize 2.0mm to 4.0mm pin shanks in positive or negative thread pin designs. This means that 2.0mm shank, small, small-plus, and medium INTERFACE™ half-pins and CENTERFACE™ full-pins in standard thread versions can be utilized directly through the clamp. Medium, positive thread pins in cancellous versions (3/16” thread diameter) will not pass directly through the small SK™ clamp, but the shank can be effectively gripped. Cancellous pins are usually placed as one of the original pins in the frame and the clamp applied to the pin; therefore, use of medium cancellous pins is simple. The small SK™ clamp can grip pins with 4mm shanks; however, as a practical matter, pins that large belong on a larger clamp and rod. The large SK™ clamp is designed to handle 3.0mm shank, medium, medium-plus, and large ESF pins, both positive and negative thread pin designs. These sizes of INTERFACE™ and CENTERFACE™ fixation pins can be applied directly through the clamp bolt, with the exception of the large cancellous version of positive thread pins - which are usually placed prior to application of clamp and rod. The maximum pin shank that can be held by and passed directly through the large SK™ clamp bolt is 3/16” (4.8mm). Human pins in the range of 3.0mm to 4.5mm can also be utilized.
Are through-and-through (full) pins no longer utilized? While dependency on full-pins is greatly reduced, they can occasionally be assets in challenging clinical cases, especially when combined with a strategy for staged disassembly. A review of mechanical testing confirms that simplified full-pin frames (only one full-pin per major fragment) are very powerful with the SK™ ESF System. These frames are readily applied to the tibia. In addition, the distal humeral condyle remains a location in which full-pins are frequently used. Due to the anatomy of the radius, full-pins are rarely recommended regardless of the ESF device utilized. Type 1-b frames are a very powerful alternative to multiple full-pin frames, providing potential for increased pin number in short fragments while utilizing safe pin corridors. Sales of full-pins to users of the SK™ ESF System have decreased significantly, while sales of half-pins have increased. This altered use ratio is supported by mechanical testing and practical results. With any ESF device, mechanical demands for stability must be weighed, and the fixation must be customized to the patient based on a fracture patient assessment, so across-the-board frame recommendations are not a reality. Patients with injuries to multiple limbs or other complicating factors will require increased (initial) frame rigidity.
I never utilize large ESF pins - only small and medium. Why should I consider the large SK™ ESF System? Frame simplification results in reduced surgery time and hardware cost. The medium 1/8” (3.2mm) pin has certainly been the workhorse over a variety of different patient sizes and readily functions with small and large SK™ ESF clamps. While the medium KE device is capable of supporting fracture healing in large and giant breeds, the resulting frames are typically very complex and hardware-intensive. While any device can be pushed to support larger patients, it is easier, quicker, and less expensive to construct a simpler frame of stronger components. This is especially true with fractures of the humerus and femur where Type I-b, Type II, and Type III frames are not possible. Staged disassembly or rod downsizing can address potential concerns about excess frame stability.

SK™ External Skeletal Fixation System
Tell me more about the economics of simple SK™ frames. Full-pins cost more than half-pins and require a clamp on each end. For example, a 6 pin, Type II ESF frame will require 12 clamps and two rods, while an 8 pin, Type I-b frame will require only 8 clamps and two rods. An even larger rod device might function in the same case as a 6 pin, Type I-a frame requiring only 6 clamps and a single rod. The 8 pin, Type 1-b frame will be easy to destabilize to a 4 pin, Type I-a if desired, and the large SK™ Type I-a frame can be easily downsized to a small SK™ rod and clamps if desired. The lack of need for aiming devices, torque wrenches, and rod augmentation further reduces cost of ownership and use. Why are carbon fiber rods and titanium rods choices with the small SK™ fixator? The 6.3mm carbon fiber rod is about the strength of the medium KE rod, and the 6.3mm titanium rod is just less than twice the strength of the medium KE rod. Since either rod is much stronger than the small KE rod, the lightweight, radiolucent carbon rod is virtually always used when pin choices are 2.0mm shank, small, or small-plus diameters. When medium pins are selected, less challenging, lighter weight patients may utilize carbon rods. On lower limbs, Type I-b or Type II minimal frames will extend the upper weight range for carbon rods. For larger patients with medium pins, the added strength of titanium rods is advantageous, especially if attempting to simplify frames or in most fractures involving the humerus or femur. At some point, more complex frames need to be built or large SK™ ESF rods should be utilized. Both carbon and titanium rods are in the small SK™ starter kit, but customers usually re-order their ultimate preference. Differences in strength can be utilized as part of a plan for staged disassembly or rod downsizing. Why are carbon fiber and aluminum rods available for the large SK™ ESF System? For the most part, aluminum rods are merely a back-up material for large SK™ rods in the unlikely event that carbon material is temporarily unavailable. Due to the 9.5mm diameter, both are much stronger than corresponding medium and even large KE rods and can serve to simplify the ESF method quite well. Down-staging from 9.5mm carbon to 9.5mm aluminum in the large SK™ is not typical and less likely to be beneficial as compared to downsizing to a small SK™ rod. How are various SK™ rods cut to length? Carbon fiber rods are not to be cut with bolt cutters or pin cutters; however, they can readily be sawed with a finetoothed hacksaw blade or an abrasive disc on a Dremel tool. Carbon fiber dust is potentially dangerous, so it is recommended that a damp paper towel be laid over the cutting area to capture any dust. Small SK™ titanium rods can be cut with a large bolt cutter, but tend to develop a significant burr that must be smoothed with a file or sanding material. Large SK™ aluminum rods can be cut with a large bolt cutter or hacksaw. All SK™ ESF System connecting rods are stocked in length increments of 50mm. The variety of lengths available should greatly reduce the need for cutting rods. Which rod should be used to construct contoured rod frames? The mini SK™ 3.2mm (1/8”) stainless steel rod is the same rod as the small KE system and is easy to contour. The small SK™ 6.3mm titanium external rod is the only rod other than the mini stainless steel rod that can be bent for this purpose. It is quite difficult to bend and is best bent prior to sterilization using a vise and two hand chucks. Plate benders can also be utilized. A different strategy must be used with large IMEX™ SK™ clamps because carbon fiber composite rods cannot be contoured. Readily constructed circular/SK™ hybrid frames, stable double clamps, and other alternatives to rod bending usually result in fixation superior to contoured rod frames.

SK™ ESF Q & A What is a simpler and more stable method of forming an adjustable articulation using SK™ components? A popular technique that has been successful for spanning joints with all sizes of SK™ frames is the utilization of modified single SK™ clamps to form an adjustable articulation that is quite secure. The SK™ single clamp body is comprised of two different parts (see page 1 for SK™ ESF clamp design): a B-1 body part that is placed on the side closest to the pin-gripping portion of the primary bolt with a threaded hole for the secondary bolt, and a B-2 body part that is placed on the opposite side with a smooth hole for the secondary bolt. Modified clamps for transarticular articulations are built with two B-2 body parts, two primary (pin-gripping) bolts, and two nuts. The articulation is built with two of these modified clamps and two short pieces of appropriate size IM pin stock or KE rod. The angle of this articulation is adjustable, and can be used to the surgeon’s advantage when a transarticular fixator is employed in the management of Achilles tendon repairs.
Which size SK™ ESF components should be used on a given patient? This is the most frequently asked question regarding any ESF device. Bone size should be examined to determine appropriate pin diameter, and pin diameter should be approximately 25% of bone diameter. Pin choice is then considered with the overall clinical demands of the patient, other injuries, and many other factors to determine not only device size, but the overall frame geometry. As previously discussed, undersized devices can be constructed to have great strength, but often become cumbersome and expensive. The better alternative is to increase inherent device strength to maintain a simpler, less expensive frame construct. For a highly comminuted, unstable fracture in a medium/large dog, large SK™ frame components and medium INTERFACE™ and CENTERFACE™ pins might be used to construct the initial frame. At approximately six weeks after surgery when staged disassembly is appropriate, small SK™ components can be substituted for large SK™ components as one method of reducing the support provided by the frame.

In addition to removing frame elements (i.e. conversion of a Type II or a Type I-b to a Type I-a), carbon fiber composite rods can be substituted for titanium rods as part of the staged disassembly of a small SK™ fixator. These recommendations are general guidelines only. On radius/ulna fractures, it is often prudent to consider the use of a Type I-b frame when unsure. With tibial fractures, also consider the use of a Type I-b frame or a minimal Type II. With fractures of the humerus/femur, it is best to use larger frame elements when unsure. These more aggressive strategies will increase the potential need for staged disassembly but will support early weight bearing and function.
Is ever-increasing stability necessary? No, we are not always looking to build the strongest ESF frame, but desire a clinically appropriate fixator that maintains a simple frame geometry that is economical, biologically friendly, and requires minimal instrumentation.
Tell me more about staged disassembly and rod downsizing. Historically, as veterinary surgeons evolved from using simple KE frames to frames utilizing multiple full-pins, the axial stiffness of the frames increased dramatically. Since this high level of axial stiffness sometimes slowed bone healing, it became popular to convert complex frames to less stiff frames as early stages of healing occurred. This sequential frame disassembly may be done in one step or in several and is termed staged disassembly. The planned reduction of fixator rigidity transfers more of the load-bearing forces across the bone, stimulating callus maturation and the later stages of bone healing. Common examples of converting a complex frame to a simpler, less rigid frame include: conversion of a Type III frame to a Type II frame, conversion of a Type II frame to a less complex Type II frame, or conversion of a Type II frame to a Type I frame. Other examples include conversion of a Type I-b frame to a Type I-a frame or removal of articulations between different components of a bi-planar frame. If one begins with a simple Type I-a frame, as is frequently possible using the SK™ ESF System, the previously listed options for staged disassembly are not applicable; however, two alternate strategies can be utilized. If reduction in pin number might jeopardize adequate pin bone interfaces; substituting a smaller, more flexible external rod for a larger, stronger one becomes a very attractive option to decrease the stiffness of a Type I frame (e.g. removal of large SK™ clamps and a 9.5mm carbon fiber composite rod and replacing them with small SK™ clamps and a 6.3mm titanium rod). While not truly a disassembly, rod downsizing does achieve the purpose of transferring a greater percentage of the load-bearing forces back to the bone and across the healing callus. A variation of this concept when utilizing the small SK™ device with 6.3mm titanium rods, is to replace the titanium rod with the less rigid carbon fiber composite rod of the same diameter. While not “downsizing” the connecting rod, this method does achieve a similar planned decrease in rigidity and might be useful in dogs when initial construction utilized medium fixation pins. Since each SK™ clamp is designed to grip a wide range of pin diameters, and there is an overlap zone between the different sizes of fixation pins gripped by the different SK™ clamp sizes, it is frequently possible to construct the initial fixator with the larger clamps and rods and replace these components at about six weeks with those one size smaller. This wide range of pin shank diameters that can effectively be gripped with the SK™ clamp makes utilization of “overlapping pin zones” with the SK™ device particularly beneficial.

SK™ ESF Q & A Not all fixator frames will require staged disassembly. In particular, young patients tend to produce bony callus rapidly and need or benefit less often from staged disassembly. All patients will benefit from early fracture stability which promotes fracture zone debridement, revascularization, and early callus formation. Only after these stages occur will the potential benefits of decreased rigidity become pertinent. With several options for converting more complex frames to less complex frames, or downstaging larger stronger rods to smaller less rigid ones, it is prudent to initially use the stronger choice with a staged exit strategy available. In skeletally mature canine patients, the optimal time period for initiating staged disassembly appears to be at about 6 weeks after surgery. 1. Pin Size versus SK™ Clamp Size This chart indicates what pins can be utilized using through-the-clamp application methods with the corresponding SK™ ESF clamp. 2. Staged Disassembly Strategies Relative to Size of Pins Used to Build SK™ Fixator This chart indicates what pins are conducive to staged frame downsizing when utilizing small or large SK™ ESF frames for initial construction. When performing this type of destabilization, pins are already in place when clamps and rods are exchanged. Since positive thread pins do not need to pass through the clamp, the range of pins amenable to this form of staged disassembly is greater than that of initial frame construction.

Basic Application Steps Do the basic application steps of the SK™ ESF device differ from the KE device? No, the basic application steps are the same; however, using SK™ components makes frame construction much easier due to the ability to add or subtract clamps at any time and place multiple pin sizes directly through the pin-gripping bolt. In addition, the secondary bolt is designed to hold the clamp in place during pre-drilling and pin placement thus assuring that pins are accurately placed through the pre-drilled hole.
What is the first step in application of the ESF frame? After approximate reduction of the fracture, a single pin is placed in the major proximal fragment and a single pin is placed into the major distal fragment. All fixator pins are place through liberal release incisions. A connecting rod is secured to these first two pins using SK™ single clamps. One can pre-place empty clamps onto the rod (faster) or place them one at a time later (less crowded.) Accuracy of reduction is rechecked and adjusted if necessary. Some difficulty in maintaining reduction may occur until additional pins are added to stabilize the environment.
Is there a sequence and technique for placement of subsequent pins? Typically, the third and fourth pins placed into an ESF frame are the two pins on each side and closest to the fracture. This is not mandatory; however, it is easier to verify accuracy of reduction and insures good pin placement. These pins are placed via liberal release incisions and through pre-drilled holes. Placing the SK™ clamp over the area of soft tissue release, and using the secondary bolt to secure it in perfect alignment for the desired drill hole and pin position, ensures correct targeting and pin placement. Next, the appropriately sized drill sleeve is passed through the primary clamp bolt to protect the soft tissue as the bone is pre-drilled. One must be careful not to crush the thin walled drill sleeve by tightening the primary clamp bolt more than just enough to hold the sleeve in position. Pre-drilling is performed through the drill sleeve while exercising care not to exert undue pressure on the drill bit. Excess pressure results in drill bit flexing and a sudden “push through” of the drill bit into the opposite soft tissues as it breaks through the bone. The drill sleeve is then removed and low speed insertion of the fixator pin is performed. After placement of pins three and four, it is recommended to recheck reduction and alignment before placing additional pins. Repeat this procedure for as many additional pins as desired.
What is the proper way to tighten fixator nuts and bolts? Tightening torque has the potential to cause loss of fracture reduction or alignment – especially early in frame construction when pin numbers are minimal. Applying minimal torque to the pin bolts early in frame design followed by increases as subsequent pins are placed can minimize this potentially disruptive force. Tightening torque can be neutralized during clamp tightening with use of the “two-wrench technique.” An open-end wrench placed on the flat surfaces of the head of the primary bolt, or on the flats of the clamp body, is used to neutralize torque force while a second wrench is used to tighten the clamp. After complete frame construction and verification of reduction accuracy, apply final torque to all bolts in an alternating fashion.

Mini SK™ Distraction/Compression Body and Rods When assembled with special right-handed and left-handed threaded rods, the Mini SK™ distraction/compression body functions as a linear distraction or compression device compatible with mini SK™ ESF clamps and pins. SK™ Hybrid External Skeletal Fixation Circular fixators typically utilize fine wires placed under tension instead of normal diameter half-pins and full-pins. When loaded, fine wires under tension behave in a non-linear fashion demonstrating low stiffness early, but increasing in stiffness as loading becomes greater. This non-linear stiffness is considered the basis of the Ilizarov method, and some consider it to enhance the fracture healing process. When components of traditional linear ESF frames and circular ESF frames are combined, the resulting frame is considered a hybrid ESF frame. Something as simple as substituting traditional pins for fine wires in a multi-ring circular ESF frame becomes a hybrid frame by definition. This is commonly done with circular ESF frames applied to the tibia to eliminate the morbidity often associated with fine wires near the stifle. By utilizing hybrid rods, the IMEX™ SK™ fixator is easily coupled with circular ESF devices to form hybrid frames that consist of a single ring with fine wires coupled to what appears to be a Type I-a or Type I-b ESF frame. These “simple” hybrid frames are relatively inexpensive and straightforward to construct as compared to more complex circular ESF frames or more complex hybrid frames. These hybrid frames are practical alternatives to transarticular ESF frames for stabilization of very small fracture fragments. In addition, hybrid frames can be alternatives to complete circular ESF frames for certain selected growth deformity patients. Due to customer requests, IMEX™ has developed a Hybrid ESF Starter Kit for construction of hybrid frames using SK™ components with our standard or mini circular ESF equipment. These kits were developed for surgeons with existing SK™ ESF component inventory and experience but not for those with significant inventory of circular ESF components. These kits contain a large variety of components to allow construction of advanced hybrid frames or multiple hybrid frames.
SK™ Hybrid ESF FAQs What are common situations where hybrid external fixators might be utilized? Since use of small diameter wires allows capture of small bone fragments, hybrid ESF frames are popular for fixation of fractures with a short juxta-articular bone fragment where standard ESF pins or bone plating might be difficult. In addition, many referral practices are finding hybrid frames very useful for correction of selected growth deformities.
Are small diameter wires always utilized across the ring when hybrid frames are constructed? No, especially when repairing juxta-articular fractures of the distal humerus or distal femur it is common to use standard full-pins and half-pins. In these locations it is difficult to obtain wire divergency because of the patient anatomy. When wires do not have divergency, less stability is achieved. Attempts to increase wire divergency in these locations results in unacceptable soft-tissue morbidity. Standard threaded pins work well in these locations.

What is the difference between “stopper” or “olive” wires and smooth wires? Small diameter wires are not threaded and therefore are subject to micromotion. When four or more smooth wires are used with good divergency, stability is adequate. When wire number is reduced or divergency is not adequate, surgeons choose to place stopper wires to reduce micromotion and enhance stability. Stopper wires have a centrally located expanded bead designed for placement against the bone. Most situations where hybrid frames are chosen will benefit from the added security provided by stopper wires.
What size SK™ linear ESF components can be used to construct hybrid frames? Mini, small, and large SK™ clamps can be utilized to construct hybrid frames. The SK™ ESF System is the most modular fixator available to veterinary surgeons and all sizes were designed to be modular. The small SK™ is the most commonly used hybrid frame due to it’s ability to cover a wide patient range. Several new components are now available to expand the versatility of the mini SK™ hybrid frame. Large SK™ hybrid frames are not often utilized unless one needs ESF pins larger than size medium.
Tell me more about tensioning fixation wires. Diameter is the key element in determining the stiffness or resistance to bending of a fixation pin. Hybrid (and circular frame) fixation wires are of small diameter, but placing them under tension can enhance their bending characteristics. IMEX™ provides a wire-tensioning device for this purpose. When the distance the wire is spanning is short (small diameter rings) or the patient is small (mini-rings) wire tensioning is not necessary. When ring diameters or patients are large, wires must be tensioned to achieve adequate mechanical stability.
What is the learning curve for hybrid external skeletal fixation? Assuming one is knowledgeable and experienced with linear external skeletal fixation, the learning curve is relatively short for fracture repair using hybrid frames. The necessary details to master are proper wire placement, wire tensioning, and frame component use. One should be well versed with linear fixators before attempting hybrid ESF. Many consider hybrid ESF a simpler alternative to growth deformity repair than true circular ESF frames. This may be true in cases that do not require post-operative lengthening, but an operative plan must be devised to help make the appropriate frame choice.
What are some other mechanical considerations to consider with hybrid frames? True circular ESF frames are built with three or four threaded rods spanning the length of the involved bone and supporting the ring in several places. Hybrid frames often mimic a type I-a or type I-b frame and only support the ring at one or two locations. With 35mm, 45mm, or 50mm rings, a single support area may result in a stable frame. When larger rings are used or unstable scenarios are encountered, it is necessary to use struts or additional sidebars to stabilize the ring in two or more locations.
What are the economics of hybrid external fixator frames? Most hybrid conversations begin with the assumption that the surgeon owns basic SK™ linear external fixation components. Therefore, only a minimum of circular fixation components is necessary as compared to a complete circular external fixation frame. Hybrid frame cost is significantly less than a complete CESF frame. The modular nature of the IMEX™ system allows one to upgrade to a true CESF frame with the same components.

Kirschner-Ehmer (KE) External Skeletal Fixation
In spite of limiting mechanical properties and the necessity for complex frames, the Kirschner-Ehmer fixator has a well-documented history in veterinary orthopaedics. Due to the widespread availability of KE components in many practices, IMEX™ continues to produce this fixator. However, the necessary frame complexity and hardware requirements make the KE device less favorable economically, mechanically, and clinically than the SK™ System by IMEX™. It is highly recommended that SK™ components be given priority for initial inventory or upgrade purposes.

IMEX™ Miniature Circular External Skeletal Fixation System Due to the increasing popularity of our miniature circular external fixator, IMEX™ has developed several new mini CESF/Hybrid CESF components to expand the capability of these devices. 1mm stopper wires with a trocar point are most often utilized for secure fixation, while 0.035” and 0.045” k-wires are also used as smooth fixation wires. All rings are aluminum and can be cut at holes for conversion into arches or partial rings as desired. Use of a wire tensioner is contraindicated with miniature rings. Mini hybrid rod supports construction of hybrid frames with mini SK™ ESF components, and mini circular ESF kits have been expanded to include these items. Rods and fasteners are stainless steel.

History of ESF Pins Veterinary external skeletal fixation developed around the Kirschner-Ehmer ESF clamp and began with the use of smooth fixation pins typically fashioned from trocar pointed Steinmann pins. Veterinary surgeons quickly discovered the necessity of threaded ESF pins and began to utilize partially threaded Steinmann pins as ESF pins. However, these pins often broke at the thread/non-thread junction and they were not considered to have adequate fatigue strength for veterinary patients. Fully threaded IM pins were then tried in an effort to eliminate the stress riser between the threaded and smooth portions of Steinmann pins, but were deemed not to be of adequate bending strength. Kirschner Medical then provided the veterinary surgeon with the Ellis™ pin (Kirschner Medical, Timonium, MD). The Ellis™ pin was basically a partially threaded IM pin with a very short length of thread. Theoretically, the short thread length would result in the thread/non-thread junction being placed in the medullary cavity and would thus be shielded from cyclic loading and fatigue failure. The resulting application would result in only the far cortex being engaged with threads and the near cortex being engaged by the smooth portion of the pin. Reports of fatigue failure were reduced but not eliminated. The near cortex was potentially stripped as the threaded/non-threaded junction passed through the wall of the bone, and potential for premature pin loosening was considerable. Veterinary surgeons then began to search human orthopaedic catalogs for strong pins of appropriate diameter to function with the Kirschner-Ehmer clamp used in many veterinary practices. The Turner Hip Pin (Zimmer) and the Centrally Threaded Traction Pin (Synthes) were two human pins that made their way into veterinary literature. A unique feature of these two human threaded pins is the presence of raised (“positive” or “rolled on”) threads. Clinical performance of these pins was considered excellent, and the “positive thread” design was considered responsible for good fatigue performance combined with adequate bending strength. These pins also provided threaded engagement for both bone cortices. Drilling of a pilot hole for placement of an ESF pin (pre-drilling) then became recognized as highly beneficial in veterinary patients. The reported performance of these human pins (two cortex, positive thread ESF pins) led to their popularity in veterinary referral centers, but limited size availability and the high cost of human implants restricted widespread use of these pins in veterinary patients. IMEX™ Veterinary, Inc. was founded to produce these pins in a variety of sizes appropriate to existing veterinary fixation devices. The resulting INTERFACE™ fixation half-pins and CENTERFACE™ fixation full-pins became available; sized for the small, medium, and large KE clamps at a cost only slightly more than that of single cortex, negative threaded pins. To complement the full versatility of SK™ ESF clamps, INTERFACE™ and CENTERFACE™ pins of intermediate sizes (2.0mm, small-plus, 3.0mm and medium-plus) are available as well. IMEX™ offers more pin diameters, lengths, and thread choices than any other veterinary orthopaedic company. Due to the popularity of these threaded pins, IMEX™ now has a selection of more than sixty ESF pins available to the veterinary surgeon, and remains the world leader in veterinary ESF pin technology. Due to the long history of the KE clamp, the terms “small, medium, and large” typically refer to pin shanks of 3/32”, 1/8” and 5/32” respectively. IMEX™ INTERFACE™ and CENTERFACE™ fixation pins do not feature the thread profile of a bone screw but are designed with the unique environmental and mechanical differences between a bone screw and ESF pin being taken into design consideration. This design stresses long-term performance of the pin-bone interface over acute performance. All IMEX™ ESF pins are produced from the highest quality, human implant grade 316LVM material.

Miniature IMEX™ INTERFACE™ Fixation Half-pins These small pins are K-wire size versions of our popular INTERFACE™ fixation half-pins. Most commonly used with acrylic frame fixators on toy breeds, avian patients, and small exotic pets, these pins have trocar points on both the smooth and threaded ends. The central area of the pin shank is factory-roughened to enhance the interface between the acrylic frame and pin. Miniature IMEX™ Stick-pins
Over the years, many customers have used IMEX™ miniature INTERFACE™ fixation half-pins for a variety of nonexternal fixation purposes when a k-wire sized implant with a short threaded portion is what the surgeon really desires. Such applications are often in small dogs, cats, avian, and exotic patients and include: tibial crest translocations, capital physeal fractures and avulsion fractures of the greater trochanter. These applications often involve bending the pin over as in tension band applications. When miniature INTERFACE™ pins are bent acutely at their roughened shaft area they may break. Stick-pins are nearly identical to miniature INTERFACE™ pins but do not include the roughened shaft that enhances grip to acrylic and epoxies, and are not prone to failure when bent acutely at the smooth portion of the pin. INTERFACE™ NP (No Point) Fixation Half-pins Standard (cortical) Thread Profile INTERFACE™ NP fixation half-pins lack the trocar point found on standard INTERFACE™ pins. Pre-drilling is required for pin placement. As with application of any ESF pin, attention should be given to proper pin alignment with the pre-drilled hole. Use of the SK™ ESF clamp’s secondary bolt will keep the clamp and drill hole oriented. In addition, liberal soft tissue release and retraction will simplify pin placement by allowing visualization. Absence of a trocar reduces opportunity for morbidity created by a trocar point remaining in soft tissue. This trocar-free pin is also safer for the operating surgeon. The lack of a trocar point makes this pin less expensive than trocar pointed INTERFACE™ pins. Lengths, diameters, and recommended drill bits are common with standard INTERFACE™ pins. SCAT™ Fixation Half-pins (Single Cortex Negative Thread) These pins are similar in design to the Ellis™ pin (Kirschner Medical) and were originally designed for use with the corresponding small and medium KE clamps. With the advent of modern ESF systems and the availability of inexpensive INTERFACE™ and CENTERFACE™ fixation pins, use of these pins is rapidly declining. Interlocking Nail Pins Interlocking Nail Pins are designed to give surgeons the ability to combine the IMEX™ SK™ ESF device with the interlocking nail system by Innovative Animal Products. Utilizing these locking pins through the nail provides a significant increase in stability that can be especially useful in mechanically challenging fracture scenarios. Used with care, these drill bits NEW 32062 SIZE tend to drill accurately through the drill sleeve and the targeting jig of the interlocking nail system. In order to support ESF pin pre-drilling through drill sleeves and with double rod drilling techniques, IMEX™ drill bits are considerably longer than standard bone plating drill bits. Surgeons must be aware of this length and avoid side pressures resulting in drill bit flexion and bowing– especially when not using drill sleeves. StickTite™ drill bits are designed to drill with minimal forward pressure and if forced into the opposite bone wall, they will engage rapidly with potential for damage to the bit. Suture Anchors Often termed tissue anchors or bone anchors, these implants are practical alternatives to bone tunnels, screwwasher combinations, and other methods of attaching soft tissue to bone. Anchors are easy to use in locations where bone tunnels are not readily placed and represent a smaller profile than screw/washer combinations. Developed in conjunction with several referral surgical facilities, the anchors and related suture materials should be considered temporary fixation devices that allow tissue to heal to its original location, protect primary soft tissue repairs during the early phases of healing, or facilitate the formation of fibrous tissue to assist in maintaining joint stability. IMEX™ suture anchors are threaded implants designed to maximize implant/bone interface by being screwed into bone. Pre-drilling is required; however, the anchors are self-tapping. Recommended drill bit diameters are 2.7mm for the 4.0mm anchor and 3.5mm for the 4.7mm anchors. Smaller drill bits can be utilized in soft, juvenile bone locations if desired. In very dense bone, slightly larger drill bits might be considered. The use of StickTite™ drill bits enhances accuracy of drill hole locations. The suture anchor’s square drive-head with cross-hole is approximately 2.0mm in height.
Suture Anchors Suture anchors are screwed into bone to act as insertion points for sutures used in reconstruction of ligaments, joint capsules and tendons. For size reference only, 4.0mm anchors will hold most brands of 60# nylon leader, and 4.7mm anchors will hold most brands of 80# nylon leader. Suture Anchor Clinical Case Examples Reconstruction of Collateral Ligaments Collateral ligament injuries are common in small animal patients. Traumatic injuries can result in tears of the ligaments, avulsions of ligament origins/insertions, or loss of ligaments from shearing injuries. Torn collateral ligaments are often sutured for primary reconstruction; however, due to the slow healing and remodeling of collateral ligament tears, it is common to utilize sutures and anchors to support a primary repair. Placing a suture anchor at origin and insertion of the collateral ligament and spanning the two anchors with a relatively strong strand of suture material is a typical method of supporting primary ligament repair. Collateral ligament avulsions are repaired by placing a single suture anchor at the avulsion site and then reattaching the ligament to the bone via the anchor. Some surgeons will elect to include a second anchor and suture loop to create a prosthetic ligament to protect the avulsion site during the early healing phase (similar to protection of a primary repair.) Injuries, such as shearing injuries, resulting in actual loss of the collateral ligament, carry a more guarded prognosis. However, suture anchors can be utilized to facilitate placement of prosthetic ligaments that serve to maintain joint stability during the formation of functional fibrous tissue, which helps maintain joint stability over time.

Suture Anchor Clinical Case Examples
Stabilization of Shoulder Subluxation and Luxation Primary, traumatic luxation of the shoulder joint is not common in small animal practice, but can occur. Congenital and developmental shoulder subluxation and luxation occur in small-breed dogs, and chronic shoulder instability occurs in large-breed, athletic dogs. Surgical stabilization for these cases can include joint capsule imbrication and prosthetic suture placement to provide reduction and strength for supporting tissue healing and strengthening of the rotator cuff. Suture anchors can be utilized for attaching these periarticular sutures. Anchors can also be used for reattaching avulsed joint capsules or glenumeral ligaments to bone.

Joint Capsule Reattachment With traumatic joint luxations, the joint capsule is always torn to some degree, and re-establishing its integrity and function is a primary goal of treatment. In some cases, the damage to the joint capsule is in the form of avulsion from the bone. In these cases, reattachment of the joint capsule to the bone increases the mechanical stability of the repair and may lead to more rapid return to function. Use of anchors in this fashion greatly facilitates re-attachment of the avulsed joint capsule by allowing the exact number of anchor points to be utilized. In comparison to more mechanically demanding scenarios, relatively small suture materials can be utilized for joint capsule reattachment. The photo to the right shows a reduced elbow luxation with avulsed joint capsule. Three suture anchors have been pre-placed into the ulna and will be used to re-position the joint capsule.

Capsulorrhaphy for Hip Luxation A number of methods for maintenance of surgical reduction of hip luxations are currently popular and include: the toggle pin method, ilio-femoral sutures to limit external rotation of the hip, and caudo-distal transposition of the greater trochanter. Each of these methods depends on the joint capsule and associated muscles for acute and long-term maintenance of joint stability. If the joint capsule is severely traumatized and not conducive to primary repair, some surgeons will elect to perform a capsulorrhaphy or dorsal augmentation of the joint capsule. This is accomplished by placing sutures from the dorsal acetabular rim to the proximal femur. These sutures are utilized to augment or mimic the strength of the dorsal aspect of the joint capsule with the hope that this added strength will assist in maintaining joint integrity while healing of the compromised joint capsule occurs. Ilio-femoral sutures can be a part of this dorsal augmentation, or can be utilized alone. The purpose of the ilio-femoral suture is to limit external rotation of the hip and thus decrease likelihood of repeat luxation. With any of the just mentioned surgical methods for stabilization of hip luxations, it is important to remember that the use of sutures and anchors must be considered temporary solutions until the joint capsule and periarticular soft tissue can heal. As such, patients with poor hip conformation are not good candidates for these methods of repair and should be considered for salvage procedures, such as FHNE or THR.

Suture Anchor Clinical Case Examples Tendon and Ligament Reattachment Other tendon and ligament injuries, such as the origin of the gastrocnemius, long digital extensor tendon of origin, and gluteal tendons of insertion may provide ideal indications for the use of suture anchors. In these cases, anchors are used in a fashion similar to treatment of collateral ligament injuries. Treatment of calcanean, patellar, and triceps tendon injuries may be successfully treated using suture anchors in very select cases. However, their use is not generally recommended for these structures due to the very dense bone at the attachment sites, the anatomical considerations unique to these sites, and the availability of other techniques which may be more appropriate for these problems. Anchor-Orthopaedic Wire-Polymethylmathacrylate-Acetabular Fracture Repair Articular fractures should be anatomically reduced and rigidly fixed. With acetabular fracture repair, it is often difficult to contour available bone plates to maintain accurate, anatomic alignment as screws are tightened into the plate. In other words, if the plate is nearly contoured, initial screw application will appear accurate; however, as additional screws are placed and tightened, any error in plate contour will result in translation of the bone and loss of reduction. To overcome the difficulty of plate contouring, special acetabular plates have been developed. In addition, reconstruction plates have been utilized in larger canine patients. Recent reports (Lewis, et al: Veterinary Surgery. 1997 May-Jun; (3):223-34) have outlined an alternate procedure for repair of acetabular fractures in small animal patients. Specifically, bone screws or similar devices are placed along the dorsal acetabular rim and are connected with orthopaedic wire. The fracture is held in reduction while PMMA is applied over the implant/wire composite frame. The PMMA conforms to the surface of the bone and strength is achieved because the composite structure is held in place by the threaded implants. Anchors may be beneficial when used for this technique, as they provide excellent bone purchase while allowing for easy retention of the orthopaedic wire in the eyelet. It is important to note that to date, this method has been used in relatively small patients.
Stabilization of Cranial Cruciate Deficient Stifles A few recommendations are provided regarding use of suture anchors for stabilization of cranial cruciate deficiency. Many questions are being asked about the appropriateness of this use, and several general guidelines and concerns can be outlined. Surgeons who assisted in development and testing of these suture anchors were referral orthopaedic surgeons utilizing tibial plateau leveling osteotomy (TPLO) as their treatment of choice for cranial cruciate tears. However, in selected patients, suture anchors have been utilized instead of the fabella for extracapsular reconstructions. Use of suture anchors enhances more isometric placement of sutures than using the fabella; however, in large active patients, it must be stressed that combinations of techniques should be utilized. For example, a fabellar suture in addition to an anchor suture could be utilized along with capsular imbrication and/or fascia latae transfer. Remembering that all prosthetic suture/ligament applications will likely fail (this includes current fabellar-tibial suture loops), it must be hoped that peri-articular fibrosis will maintain joint stability after such failure. Perhaps the use of multiple techniques creates a situation in which multiple failures must occur over time before there is only fibrosis and fascial transfer to maintain long-term stability. More and more customers are incorporating additional use of suture

anchors as a portion of their CCL repair in challenging patients. Do not purchase suture anchors as the sole method of extracapsular repair of cruciate disease; however, selected adjunct use of these anchors may simplify and enhance cruciate repairs. Summary Many customers want specific recommendations regarding suture material type and size. Due to the widespread variations in patient size, underlying pathology, and presence or absence of infection, specific recommendations cannot be made. However, feedback to date indicates that about 50% of cases have utilized monofilament sutures, while the other 50% have utilized braided materials. In human surgery, there are dozens of suture anchors available, and many are pre-loaded with braided suture material. Large diameter, monofilament nylon fishing line has been popularized in veterinary medicine and may be appropriate in demanding situations such as repair of CCL deficient stifles in large, active patients. However, this stiff line is not conducive to acute bends at anchors and is difficult to handle. Current 4.7mm anchors will support use of most brands of 80 pound monofilament nylon leader and #2 -#4 braided materials. Number 5 braided materials may be difficult to utilize if the material of choice tends to fray significantly when cut. Popular monofilament materials also include Prolene and Ethilon in sizes “0” and “2.” Consider the difference between a closed tear of carpal collateral ligament that undergoes primary repair and prosthetic ligament support compared to a deep shearing injury loss of the same ligament in a similar patient. The shearing injury is less stable, but being open and contaminated may lead one to utilize monofilament material for repair. Size of material selected is dependent on patient size. Braided material tends to have better handling and tying characteristics, is more conducive to sudden direction change, and is generally stronger than monofilament material. However, in veterinary medicine, both have been associated with fistulous tracts. Use of higher quality braided material and improved aseptic techniques seem to be leading to increased use of braided suture material in veterinary surgery. Newer monofilament, non-absorbable sutures show promise as well.

The Toggle Pin Method of Coxofemoral Luxation Repair Introduction There are a number of currently popular surgical methods for maintaining reduction of coxofemoral luxations that include: the toggle pin method, ilio-femoral sutures to limit external rotation of the hip, and caudo-distal transposition of the greater trochanter. Each of these methods depends on the joint capsule and associated muscles for acute, and especially for long-term maintenance, of joint stability. If the joint capsule is severely traumatized and not conducive to primary repair, some surgeons elect to perform a capsulorrhaphy or dorsal suture augmentation of the joint capsule. With any of the just mentioned surgical methods for stabilization of hip luxations, it is important to remember that the use of sutures and anchors must be considered temporary solutions until the joint capsule and periarticular soft tissue can heal. As such, patients with poor hip conformation are not good candidates for these methods of repair and should be considered for salvage procedures, such as FHNE or THR. The toggle pin method of maintenance of coxofemoral luxations has been around many years. Commercially available toggle pins, IMEX™ Polypropylene Suture Buttons, and the IMEX™ Universal Aiming Device have simplified the method and increased its popularity. This method places a strand (or multiple strands) of suture material in a location that mimics the normal origin and insertion of the round ligament of the femoral head, which is torn when the hip is traumatically luxated. This anatomic positioning of suture material is relatively straightforward and visually demarcated in the hip, and perhaps is partially responsible for the logic and popularity of the method. Different suture materials and sizes are used based on the surgeon’s preference. IMEX™ customers use both monofilament and braided suture materials. Braided material is popular with customers who desire strength and knot security and tends to bend acutely around anchors or toggles better than monofilament suture material. Typical sizes include #2 and #5. Prolene®, Ethilon® and monofilaments are popular with customers who desire a relatively inert material, and common examples include monofilament fishing leader in 40 lb., 60 lb., and 80 lb. tests.

Surgical Procedure Figure 1: This method of repair is best accomplished through a craniolateral approach to the hip with caudal retraction of the femur to allow for complete visualization, exploration, and appropriate debridement of the acetabulum. After debridement of impinging tissue, clot, and remaining round ligament in the acetabulum, a hole is drilled completely through the medial wall of the acetabulum centered in the acetabular fossa. Use of an A/O type, handheld drill sleeve will simplify drilling while protecting the femoral head. The diameter of the hole must be large enough for the toggle pin and suture combination chosen. A 3.2mm toggle pin will require at minimum a 3.5mm hole. However, a 4.0mm hole or larger is often needed if using heavy monofilament line. For the 4.0mm toggle pin, a 4.5mm or 5.5mm drill bit is recommended. The toggle pin-suture combination should be easily inserted through the acetabular drill hole. If difficulty is encountered, the acetabular hole should be enlarged with a larger drill bit.

Figure 2: The aiming device is used to drill a femoral tunnel for the suture. The aiming device is placed so the tunnel is drilled from the subtrochanteric area of the lateral femur to the fovea capitis of the femoral head. It is helpful to have a surgical assistant at this time. The 3.5mm drill bit is the most Figure 2 common drill bit utilized and also the minimum hole diameter used with the suture passer. When drilling of the femoral tunnel is nearly complete, it is advisable to remove the aiming device and complete the drill hole by hand. This eliminates potential damage to the drill bit. Figure 3: The suture used for repair is passed once through the hole in the toggle pin creating a simple loop. The toggle pin is held at one end using large needle holders, Kelly forceps, or a similar instrument. The suture is Figure 3 pulled tight along the sides of the toggle pin so each strand seats within the toggle pin grooves. The toggle pin is then visually started into the acetabular drill hole and inserted as far as the instrument will allow. The instrument is removed, and the blunt end of the IMEX™ suture passer is used to push the toggle pin completely through the acetabular drill hole. (If this step seems to require excess force, it is likely that the acetabular drill hole is not large enough to accommodate the diameter of the toggle pin-suture combination used.) Figure 4a: The ends of the suture are spread and tensioned to pull the toggle pin tight against the medial wall of the acetabulum. The toggle pin is tested for secure seating within the pelvic canal. The suture is then pushed or pulled through the femoral canal to exit the lateral femur. The suture is tensioned Figure 4a while the femoral head is reduced into the acetabulum. Figure 4b: Appropriate reduction is verified and the ends of the suture are tied over another toggle pin or a suture button as shown in figure 4b. Alternatively, another small bone tunnel can be drilled in the lateral aspect of the femur to allow one of the suture strands to be passed through and then tied to the opposite strand. The hip should be appropriately reduced Figure 4b and firmly seated; however, do not over-tighten the suture by placing excess tension on it. This will adversely effect hip joint range of motion and will cause the suture to fail prematurely.

SK™ Range of Motion (ROM™) Hinge A recent development in transarticular external fixation (TESF) is the release of two sizes of a range of motion (ROM™) hinge. These hinges combine an adjustable joint angle into a lockable TESF hinge that can allow a variable range of motion as selected by the surgeon. Previous versions of this device have been utilized in research projects and have made their way into clinical use. Each size ROM™ hinge features a multi-toothed locking nut that allows the surgeon to lock-in any desired starting position or “normal standing angle.” The opposite end of the ROM™ hinge body features a central flat that determines the locked position, and two adjacent arcs that allow for a variable degree of motion to be dialed in as desired. The ability to progressively increase the allotted range of motion during the healing period is unique to the IMEX™ ROM™ hinge and can be utilized or ignored depending on patient management requirements. ROM™ hinges are modular by design and the hinge rods are easily interchanged to accommodate different limb length and patient anatomy. To change rods, loosen the corresponding nut or nuts and unscrew the rod from its mating hinge pin. This can usually be done by hand, but a pair of pliers can be used if needed. Since the hinge pin is held in position by the rod, it must be positioned before screwing in the replacement rod. Rod insertion into the hinge pin can be achieved from one direction only; therefore, it is necessary to seat the hinge pin properly to ensure correct rod insertion.
