FRASER GOODFELLOWS HOLIDAY ASSISTANCE 2008 For all children 17 and under

Registration Requirements 1)

PROOF OF RESIDENCY:

You MUST be a Fraser Resident and Provide: Photo ID with Current Address OR Current Utility Bill 2)

PROOF OF INCOME:

You MUST Provide: Current Pay Stub OR

Bridge Card (or proof that you are receiving aid… i.e. Free Lunch Program, Food Stamps)

PROOF OF FAMILY SIZE:

You MUST have Birth Certificate(s) for your Child(ren) OR You MUST have Guardianship Papers if you are not the parent

THE ABOVE DOCUMENTATION IS NECESSARY TO REGISTER

Thursday, November 20th Tuesday, December 2nd Monday, December 8th
