December 1, 2017 TO WHOM IT MAY CONCERN: I, the undersigned parent of Jolina Grace G. Abaga, a resident of Barangay San Vicente, San Fernando, La Union understand that the curriculum for the degree Bachelor of Science in Accountancy calls for a six (6)unit practicum equivalent to a minimum of one hundred (100) hours office training for prospective graduates. I allow my child to undertake office practice to start on Semestral Break until she has fulfilled the one hundred hours minimum requirement. I agree to the time schedule by the office where my child is assigned. I also understand that certain risks are involved during the office training. For this reason, I waive any claim against the Administration of Saint Louis College, City of San Fernando, La Union and the Office where my child is assigned from any liability arising from unforeseen event. I execute this waiver with the understanding, however, that the adviser/administrator of Saint Louis College and/or the Department of Budget and Management, Regional Office No. 1 will execute due care and diligence expected of a good parent of a family.
