Date: ___________________________ Dear Parents: Your child is eligible to receive a Fairfield City School District technology and communication services account and needs your permission to do so if he/she is under the age of 18. Your child will have the opportunity to communicate with students throughout the district and other schools, as well as organizations around the world. An Internet account allows your child the opportunity to collect and share information, research a variety of subjects, and experience a wealth of educational simulations. With this educational opportunity also comes responsibility and respect. It is important that you and your child read the enclosed Access Authorization and Release/Agreement Form and the Fairfield City School District Technology and Communication Services Policy and Regulations. It is essential that you discuss these issues with your child to ensure their understanding and accountability. When your child is given an account and password to use on the computer, it is essential that the rules be followed. Failure to do so may result in the loss of the privilege to use this educational tool. Disciplinary action is also possible. Remember that you are legally responsible for your child’s actions. Please stress to your child the importance of using only his/her own account and password. He/she should not share it with friends or permit anyone, other than his/her authorized supervisor, to use his/her account or password. Although we have established a technology and communication services policy, please be aware that if your child utilizes the Internet, it is possible to access unacceptable materials or communications. The district has taken precautions to avoid this occurrence, but it cannot always be prevented, as we cannot control other computer systems. If you agree to allow your child under 18 access to technology and communication services, please sign the Fairfield City School District Technology and Communication Services Access Authorization and Release/Agreement Form and return the form to your child’s teacher. Thank you for your support and understanding as we move forward with technology and communication services. This will provide our students with quality learning experiences. Sincerely,

Fairfield City School District Staff
