Evolution Getting Started Packet Welcome to Evolution Basketball. We are excited to have your child begin training with us. We ask you to review the information in this getting started packet to better understand how our program works. If you have any questions about the training or any other general questions please do not hesitate to ask your trainer.

Information about training for PARENTS to think about: - There are different types of training within the private and group training sessions. We conduct the training sessions with three separate aspects in mind. 1. "General Skills". We look at the general skills training as something that all players need. For example, ball handling, passing, layups, footwork, and pivoting. These are skills that are needed before moving on to more advanced skill sets. 2. "Position Specific". These are going to be the skills that are needed to play a certain position. For example, the post players will be working on their back to the basket moves, finishing shots around the basket, and boxing out/rebounding. While the guards will be doing more advanced ballhandling and passing skills, understanding attacking angles from the perimeter, and shooting off of the catch and dribble. 3. "Shooting Mechanics". Teaching shooting is extremely important in the game of basketball. The mechanics of shooting are very specific skills that take time to develop. Teaching shooting form is something that we suggest doing in private workouts. Because of the specific nature of shooting mechanics it becomes more difficult to teach the more players that are involved. Also, each kid has different problems with their shot and getting to focus just on that players mechanics can speed up the learning curve. - Youth League players generally practice once or twice a week. At this frequency, there is only so much that the players will be able to improve. Adding in a workout or two during the winter season can greatly improve the players progress over the winter season. Like anything else in life the more time you put into it, the quicker you will improve. - No matter how good a coach is at any level they cannot focus on each specific players needs in a team practice. That's where we can help develop each players individual skills. During those team practices coaches are forced to focus on running plays, team defense, etc. They don't have time to focus on the kids individual skills like their ball handling, passing, footwork, and especially shooting mechanics.

Private $300 (5 sessions @ $60/session) Partner $200 (5 sessions @ $40/session) *Siblings $400 Group $175 (5 sessions @ $35/session) *Siblings $350 Pay as you go (Private $65/hr – Partner $45/hr – Group $40/hr)

Cancellation Policy: For each 2 packages (10 sessions) you are allowed 1 excused cancellation. An excused cancellation must be made prior to 24 hours of the session date/time. All other cancellations will be charged. For clients who choose to pay as you go. There is a 24 hour cancellation policy. Any cancellations made within 24 hours of the session date will be expected to be paid prior to the next training session. ALL Cancellations must be made by a PHONE CALL directly to the trainer. Email or text message cancellations will not be accepted.

Returned Check Policy: All returned checks will be charged a $10 fee.

Parent Expectations: Parents, we kindly ask you to not coach your child during workouts. We as trainers need your child's full concentration and any outside distractions will only hinder their progress. Remember you are paying us to coach your child, please let us do our job.

(Read Carefully Before Signing) In consideration of being permitted to participate in any way in the Evolution Program indicated below and/or

being permitted to enter for any purpose any restricted area (here in defined as any area where in admittance to the general public is prohibited), the parent(s) and/or legal guardian(s) of the minor participant named below agree:

1. The parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating in the

below Evolution activity or event, he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe, the participant should immediately advise the officials of such condition and refuse to participate. I understand and agreed that, if at any time, I feel anything to be UNSAFE, I will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further. 2. I/WE fully understand and acknowledge that:

(a) There are risks and dangers associated with participation in Evolution events and activities which could result in bodily injury partial and/or total disability, paralysis and death.

(b) The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe.

(c) These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below. (d) There may be other risks not known to us or are not reasonably foreseeable at his time.

3. I/WE accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis or death, however caused and whether caused in whole or in part by the negligence of the Releasees named below.

4. I/WE HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the Evolution facility

used by the participant, including its owners, managers, promoters, lessees of premises used to conduct the Evolution event or program, premises and event inspectors, underwriters, consultants and others who give

recommendations, directions, or instructions to engage in risk evaluation or loss control activities regarding the Evolution facility or events held at such facility and each of them, their directors, officers, agents, employees, all for the purposes herein referred to as “Releasee”...FROM ALL LIABILITY TO THE

UNDERSIGNED, my/our personal representatives, assigns, executors, heirs and next to kin FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES OR DAMAGES AND ANY CLAIMS OR DEMANDS THEREFORE ON

ACCOUNT OF ANY INJURY, INCLUDING BUT NOT LIMITED TO THE DEATH OF THE PARTICIPANT OR DAMAGE TO PROPERTY, ARISING OUT OF OR RELATING TO THE EVENT(S) CAUSED OR ALLEGED

OF THE RELEASEE OR OTHERWISE.

TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE

5. I/WE HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and

involve the risk of serious injury and/or death and/or property damage. Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES.

6. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.

7. On behalf of the participant and individually, the undersigned partner(s) and/or legal guardian(s) for the

minor participant executes this Waiver and Release. If, despite this release, the participant makes a claim against any of the Releasees, the parent(s) and/or legal guardian(s) will reimburse the Releasee for any

money which they have paid to the participant, or on his behalf, and hold them harmless. I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY

SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT,
