Registration & Payment Policies Registration. New students can register for 6-week sessions before the start of the session. Student Registration Form and payment is required. Payment can be made online (See the Enso Online link on our web site) or in person (by appointment). Registrations are accepted on a first-come, first served basis. Continuing students must pay for their next session by the tuition due date. Continuing students have priority registration over new students. Late registration may be allowed depending on available space. 6-Week Karate Intro Sessions. New students--both adults and youth (age 6 and up)--may enroll in 6-week Karate Intro sessions which include two classes per week. Classes available to beginners are marked with asterisks (*) above, subject to availability. Two Karate Intro sessions are offered each session. A uniform and Student Handbook is included. Upon completion of the Karate Intro, students may enroll with the continuing karate student fees (at a prorated amount if enrolling mid-session). Prospective students are welcome to observe classes before signing up for the Karate Intro by appointment. 12-Week Karate Sessions. Continuing students enroll in 12-week sessions. See the session dates, tuition due dates, holidays, and breaks on Page 1. Classes may be canceled occasionally due to inclement weather or instructor illness. See Refunds and Absences below. Kinder Karate. Students ages 4-6 may enroll anytime during the session depending on availability. We strongly encourage early enrollment to reserve your space. Auto-Renew Option for Ongoing Registration. With the auto-renew option, we will automatically re-register the student for the next 12-week session and take payment for tuition with a credit card or checking account. This will be processed safely and securely with our online registration system on each tuition due date. The auto-renew option is not a yearly contract; students can select not to re-register with written notice to Enso at least one week before the tuition due date. This option will help students keep their accounts current and secure their spots in their classes. Refunds. There are no refunds. Payments must be made in full at the time of registration (in advance of classes). Students are not permitted to attend classes without payment. Absences. Tuition fees cover classes during the scheduled session dates. Payments must be made full regardless of the number of classes actually attended. Attending extra classes to make-up for missed classes within a session can be made with permission, depending on availability. Students expecting to be absent for four weeks or more must request a proration in advance of the expected absence. We cannot prorate tuition due to short vacations or occasional missed classes. Testing Fees. In addition to class fees, students who wish to test for rank promotion must pay a testing fee. The fee for students who are white belt through purple belt (10th - 4th kyu) is $35. For brown belt and up (3rd kyu and up), the fee is $50. Students who need to re-test do not have to pay for the re-test. The stripe testing in the Kinder Karate program does not require an additional fee. Updated January 26, 2009 Page 2

Indemnification and Release The undersigned hereby acknowledges that he/she has enrolled in a program of strenuous physical exercise and activity including but not limited yoga and martial arts, which may include strength training, stretching, aerobic activity, meditation, and various physical exercises (hereafter referred to as the “workout”). The undersigned hereby represents and warrants to Enso, LLC and its partners that the undersigned has consulted with his or her personal physician with respect to his/ her participation in the workout and that the undersigned is in good physical condition and does not suffer from any disability that would prevent or limit the undersigned’s participation in the workout; and, after having conducted a physical exam of the undersigned’s personal physician has authorized the undersigned’s participation in the workout. The undersigned fully understands and acknowledges that the workout involves strenuous physical activity and that physical injury can occur as a result of participation in the workout. In consideration of the undersigned’s participation of the workout with Enso, the undersigned, his/her heirs, successors, and assigns, hereby released and forever discharges Enso, LLC from any and all liability whether now existing or occurring in the future including but not limited to liability for heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, stress fracture of bones, shin splints, tendonitis, heat prostration knee injuries, back injuries, foot injuries and any other illness or physical injury related to or arising from or out of the undersigned’s participation in the workout, collectively hereafter referred to as the “release.” The undersigned hereby acknowledges that they have read the agreement in full and understand the release and the terms contained herein, are not now under any duress, coercion or undue influence or, under the influence of any alcohol, drugs or other intoxicating substance and hereby agree to be fully bound by the terms herein. Incident to the release, the undersigned agrees to indemnify and hold Enso, LLC harmless from any and all liability covered by the release.
