Why is your patient on this medication: Prevention of constipation in pt who should avoid straining after surgery. To counteract effects of narcotics. Mechanism of action and indications: (Why med ordered) Promotes incorporation of water into stool, resulting in softer fecal mass. Therapeutic effects: softening and passage of stool.
For IV meds, compatibility with IV drips and/or solutions: N/A
For prevention of constipation.
Common side effects: EENT: throat irritation GI: mild cramps Derm: rashes
Interactions with other patient drugs, OTC or herbal medicines (ask patient specifically)
Lab value alterations caused by medicine: Long term use may cause electrolyte imbalances
Interacts with other laxatives: MOM
Be sure to teach the patient the following about this medication: Encourage using other forms of bowel regulation: increasing bulk in diet, increasing fluid intake (6-8 full glasses/day), increasing mobility. Pts with cardiac disease need to avoid straining during bowel movements (Valsalva maneuver).
Nursing Process- Assessment (Pre-administration assessment) Assess for abdominal distention, presence of bowel sounds, and usual pattern of bowel function. Assess color, consistency, and amount of stool produced.
Nursing Implications (what to focus on): Contraindications/warnings/interactions: Hypersensitivity, Abdominal pain, N/V, especially with fever or other signs of acute abdomen. Excessive or prolonged use may lead to dependence.
Assessment Why would you hold or not give this med? Diarrhea, rectal bleeding, abdominal distention, abdominal cramps, N/V, or fever.
Evaluation Check after giving Pt should have a soft, formed bowel movement; usually within 24-48 hr. Therapy may take 3-5 days for results.
