Based on what I’ve seen, this person has consistently been involved and has participated actively in our Care Group during the entire evaluation period. B. The attitude of this person toward the group and toward what we were doing as a group was very positive. C. This person has shown a real desire to learn throughout our study of Developing Relationships, making every effort to apply what he/she is learning, not just in his/her own life, but more so to our group as a group. D. This person believes in the concept of group life so much so that I’ve seen him/her go the extra mile to make our group a strong group. E. This person has been very cooperative in whatever the group is doing, making sure that he/she is contributing positively to the group process. F. This person speaks and shares openly during group discussions, enabling the whole group to know more of what is in his/her mind. G. This person is willing to resolve conflicts whenever they happen, showing real humility and Christian love throughout the process. H. This person has shown his/her love and cares for me personally by visiting me at home/office or calling/texting me whenever the opportunity arises. I. I’ve seen this person take an active interest in the welfare of others, making an extra effort to help them or minister to their needs. J. This person has been very truthful and honest to the group, as far as I know, and I find it easy to trust him/her about anything. K. This person knows how to accept differences among people, celebrating rather than criticizing others for their uniqueness. L. I believe this person has opened his/her life to the group, such that I feel that he/she has allowed us to know more about him/her in the process. M. This person knows his/her own responsibility for his/her own choices and does not burden the group with overdependence, blaming or guilt trip. N. I’ve seen this person help carry other people’s loads without complaining or arguing. O.
