CONSENT FORM Circuit Overload Solutions I have been given information about the study “Board Wiring Problems” that seeks to identify various syntactic errors when board wiring problems occur and interpret the board wiring problems of EIM Grade 11 Students at Western Bicutan National High School of which I am willing to participate as a respondent. I have been oriented with the specific objectives as well as the potential risks of this research and given an opportunity to ask the researchers any questions that I may have regarding the study and my involvement. I understand that my participation in this study is voluntary; I am free to refuse to participate and withdraw from it anytime. Furthermore, my refusal to participate or withdrawal of consent shall not affect me personally and/or my relationship with the researchers nor any professional relations within the academic premises. PARENTAL CONSENT FORM

We, the researchers of Grade 12 EIM-02 Aquarius , are currently undertaking research studies in partial fulfillment of our requirements in Inquiries, Investigation & Immersion. In this regard, we are asking your permission for your child to participate in our study as one of the respondents. This reseach is titled “Board Wiring Problems” which aims to know what are the impact of Board wiring problems to the performance task of eim Students, what are the causes of these probelms and what are the solutions that are commonly committed by the EIM Students.Your child will be using a interview to get the data that we need from our respondents. Its is our hope that data from this instruments will contribute to a better understanding of the Board wiring problems to the performance task of EIM Students.
