CAREGIVER EVALUATION SUMMARY Caregiver being evaluated: _________________________________ Date; ___/___/___ Evaluation Criteria A. Based on what I’ve seen, this person has shown true servant leadership in what he/she thinks, says or does. B. This person knows how to minister to people who are not yet in the community or are still in the process of becoming Covenant Members. C. I’ve seen this person make time for people who are still outside the community but are already in the church, especially during the SWC, helping them to get connected with the church in different ways. D. This person makes the time to visit people in their homes and care for them, especially for those who are still outside the Care Group or Community but are already inside the church. E. This person knows how to lead the discussions during the Developing Relationships course. F. This person knows how to affirm people, making them feel important and accepted both inside the Care Group as well as in the Community. G. The Care Group is becoming more and more like a real group as time goes by because of the leadership of this person. H. This person makes the time to visit and care for those who are already attending the Care Group. He/she visits them in their homes or offices and generally makes time for personal contact with them. I. This person allows each one in the Care Group to express themselves, giving them permission to be who they are and to learn together with the group the principles of developing relationships. J. This person has been a good example and model for the Care Group in terms of what it means to be a faithful disciple of Jesus Christ. K. I am confident that this person knows the condition of his/her “flock” because he/she takes the time to know what’s going on in their lives. L. This person knows how to comfort others in time of physical need (e.g. visit and pray for them when they are sick, etc.) M. This person will sometimes go out of his/her way to assist people in their material needs (e.g. calling up others for help or raising funds for legitimate material needs) N. This person knows how to rebuke or correct people whenever necessary. He/she is not afraid of what people would say or think (i.e. he/she is not a people pleaser) O. This person knows how to identify spiritual needs and knows how to minister to people with such needs so that they can be refreshed or restored by the Spirit. P. This person is a good listener, able to encourage people in their emotional or psychological needs (e.g. a good counselor, etc.). For Improvements Strengths
