Kumari H.M.D., a short, lean and sweet child was brought to my clinic by her paternal aunt. Although she was 13, she appeared hardly 8-9 years old. She had come down from a small town in Saurashtra (Gujarat) to Mumbai for improvement in her stunted growth. Child was around 42", weighing barely 23 kg with sharp and delicate features. She was shy and soft spoken but answered all the questions put to her with a sweet smile and clarity. (Before coming to clinic aunt noticed her nervousness). History revealed that she had stopped growing after an attack of Typhoid at the age of 9, Fever (101-103 degree F) lasted for 2 months and was associated with diarrhoea, body ache, anorexia and increased thirst. It drained her energy to the extent that she was to be carried by mother during the sickness. She has not gained any weight since then, although she takes full meal with adequate ghee. With sparkle in her eyes she enumerated her cravings - MILK, ICE CREAMS, LEMON, ghee & spicy. She did not prefer much SWEETS or chocolates. Information regarding birth history and milestones could not be obtained as her mother was at village. Her aunt recollected that the child did suffer from dentition diarrhoea and probably her speech was delayed. H / O nocturnal enuresis, > since one year. Thermal state Ambithermal. Dreams: Religious of God. LIFE SPACE Family Situation: Her family consists of business man father, housewife mother an elder brother & an younger brother. She is more attached to her mother. When mothers nature was described as irritable by the aunt, she showed her dislike by looking down and denying it by saying that mother was good. Mother is fastidious and demands work in proper way. She helps mother in all house hold chores. Her sister is also fastidious. When brother and sister fights, she prefers to be away from it as she likes peace. She hardly fight with anybody. Father is a
mild person. She does not mess around the house with her belongings, rather her books and games are always arranged very neatly. She is very particular about not spoiling her things. The aunt added " her suitcase is also well arranged & maintained". She gets angry on herself if she commits any mistake. She is sensitive to reprimands and used to break into tears at trifles. But now she asks the reason for it. She is fond of music and dancing "When ever I listen to music, most of time I dance". She enjoys watching Ras-garba & magic show on TV. She becomes very anxious if mother takes ill, then she goes out of her way to comfort her. Sensitive at heart, she feels like helping when finds anyone suffering. She was good at studies and is keen on extracurricular studies and activities she is anxious before reaching stage, but then does well. She was holding 3rd 4th rank but last year she stood 23rd due to difficulty in maths and Gujarati. As such she always used to be tensed up before exams, about her performance but now since 1 year, she has fear of failure that if she fails then she would be in lower class & loose her friends. MENTAL STATE: INTERPRETATION & PSYCHODYNAMICS Repertorial approach: Mental state is having characteristic qualified symptoms, characteristic desires, aversions are available. So remaining at specific level will help to arrive at remedy easily. Such facility is best provided in Kentian way or repertorisation. Selection of repertory: Synthesis by Frederik Schroyens, Ed. 5.2" because it is updated / extended version of Kents repertory (Structure like Kents) & it gives relatively better list of remedies in each rubric. Rubrics 1. Mind Fastidious 2. Mind Fear, failure of Exams
3. Mind Anger, mistakes about his 4. Mind Sensitive to reprimands. 5. Mind cares, Full of others, about 6. Mind Sensitive to music, dance. 7. Mind Confidence, want of self 8. Generals Food desires - milk 9. Generals Food desires, Ice-Cream 10. Generals Food desires, - spices 11. Generals Food desires Lemon 12. Generals Food desires Aversion - Sweets 13. Generals Food desires Aversion Chocolates. 14. General Development arrested. Reference of above rubrics suggests group of remedies viz. Ars. alb., Calc. phos. Calc sil, Carcinosin, Causticum, Graph, Phos., Sil, Staph, etc. Silica comes closer in respect of anxiety before exams, fear of failure but performs well, shyness and fastidiousness but does not cover such qualified sensitivity for others pain, reprimands and music. Phos. Phos also covers majority of symptoms but qualitatively her sensitivity differs in a way that she is not only sensitive to pain of others pain but also to reprimands. She did not present with liveliness nor with charming relationship as seen in sentimental and sympathetic phosphorus children (she attracts attention due to shyness & politeness).
Typhoidinum is another remedy which tempts ones attention because of bad H / o Typhoid & not well since then. Kents following advice checks us from such actions where by he means that no remedy / nosode / intercurrent remedy, should be prescribed by the name of any disease / miasm / hypothesis / theories. Prescription has to be made only on principle Similia similibus curantur. Pulsatilla cover many symptoms: viz - shyness, fastidious, sensitive where by weeps easily, Av: sweet, Cr: Ice cream, spicy, lemon and dentition diarrhoea & thermal state. Examining qualities one can make out that finer aspect is missed by it. Patient shy where by anxiety for performance & nervousness are in the base not the timidity as alter on she performs wells tears mainly represents her inner hurts & reprimands but not all the emotional expression. Patients anticipatory anxiety before the task, anger on her mistake (that kind of fastidiousness) is not seen in Puls. Carsinosin matches to the portrait of this case qualitatively & quantitatively covering mental, physical & miasmatic (not well after acute illness-dwarfish) aspects. Remedy Selected: Carsinosin. Posology: Mental & Physical characteristics available, almost total correspondence to selected remedy, functional complaints, miasmatic block present. Potency selected: 1m Doses: Infrequent. 10-5-96. Treatment commenced: Carcinosin 1m 1 dose. Progress notes: Since then patient is kept on sac lac. On 25-5-96 she reported APPETITE with general feeling of well being. Then she went to village. 13-6-96" height increased by 1.5", weight by 1kg. 22-8-96: weight increase by 1kg. & reported nocturnal enuresis. As per 16-11-96 her height increased by 2" and weight by 2kg. but since 1-2 months no news. Over all patient put on 3.5" and 4 kg. Further inquiry has been made for her secondary sexual development, menses,
performance in studies, sensitivity, nocturnal enuresis etc. It has been noted that severe stress can produce more cortisones in body which can inhibit growth hormones. Response of Carcinocin in this case gives confidence that right homoeopathic remedy can provide adequate environment for bonsai to grow natural way. I would like to present concordances of Carcinocin in this case through Joslins circle. (See pg. 167) (Which can be complimentary or antidotal at a time) and less related in further circles hence follow well & inimicals can be studied.
