Budget Justification Form Salaries 1) William Loui, MD, is the Chief of Oncology at the Queen’s Medical Center (Queen’s) and will serve as the Program Director of “The Queen’s Cancer Survivorship Program for Moloka`i Breast Cancer Patients.” Dr. Loui will be responsible for all aspects of the program, including its design, implementation, and evaluation. Dr. Loui will spend approximately 5% of his time on this project. Dr. Loui’s salary will be provided in-kind. 2) Francisco Conde, APRN, PhD, is an Oncology Clinical Nurse Specialist at Queen’s and will serve as the Survivorship Navigator for this project. Dr. Conde will be responsible for the maintenance of the EQUICARE CS™ case management system and coordinating follow-up appointments and travel arrangements. Dr. Conde will spend approximately 10% of his time on this project. Dr. Conde’s salary will be provided in-kind. Fringe: As all salaries will be provided in-kind, no funding is requested for fringe. Consultant Costs: No funding is requested for consultant costs. Supplies: No funding is requested for supplies. Equipment:: Funding is requested for video conference equipment to be used for video conferences between Queen’s and Moloka`i General Hospital (MGH). It is estimated that this video conference equipment will total $2,500. In addition, funding is requested for a conference phone, which is estimated at $500. Total funding requested = $2,500 + $500 = $3,000. Travel and Conference Fees: Funding is requested for 2 physicians, 2 oncology nurses, and 3 patient navigators from MGH to attend a national workshop or conference related to cancer survivorship care. It is estimated that conference registration fees, plane and ground transportation, and lodging will cost $1,400 per person ($1,400 x 7 people = $9,800). Funding is also requested for travel costs, including round-trip airfare and taxi fare, for Dr. Conde traveling from Oahu to Moloka`i to provide training and education for physicians and staff regarding the cancer survivorship program and use of Cogent software. Round-trip estimate is $240/visit, and 6 visits will be conducted ($240 x 6 visits = $1,440). Total funding requested = $9,800 + $1,440 = $11,240. Patient Care Costs: No funding is requested for patient care costs. Sub-contracts: No funding is requested for sub-contracts. Other: Funding is requested for fees for EQUICARE CS™, which is a web-based case management system for cancer survivorship provided through Cogent Health Solutions. EQUICARE CS™ is licensed under a Software-as-a-Service (SaaS) model. Because all the initial capital investments in hardware, training, and installation services already have been paid by Queen’s, the only SaaS costs are the flat fees per patient per year, which is currently $75. Total funding requested = 100 patients x $75/patient = $7,500. Indirect Costs: Funding is requested for indirect costs associated with this project. In accordance with application guidelines, indirect costs are calculated at 15% of direct costs. Total funding requested = $3,261.
