Newborn Baby Feeding Diary Instructions: Date: Today’s date Breastfeeding: indicated the time of the feed Right and Left Breast: how many minutes did you feed on each side Urine: indicate pale, concentrated, or crystals Stool: if they made something at the time of the feed indicate dark (meconium), greenbrown (transitional), or yellow Supplemental Feed Method: if you did not breastfeed, indicated how you fed: finger feeding, bottle, feeding tube, syringe, or other Supplemental Breast Milk: note in ounces or milliliters how much your baby ate from a bottle Supplemental Formula: note in ounces or milliliters how much your baby ate Pumping: indicate total time you pumped and the amount of breast milk you collected Notes: write anything of interest such as: baby very sleepy This log is not a substitute for professional lactation advice.
