August 7, 2003 Michigan Technological University Internal Audit Department Amy L. Hughes, CPA- Director, Internal Audit An Informal Audit Report on Tuition and Fee Revenue of Fiscal Year 2003

Audit Objectives The primary objectives of this audit were to determine whether tuition and fees were administered according to University policies and whether tuition and fees revenue was fairly stated in the financial records. Testing was performed to determine if: • students were charged tuition and fees using rates approved by the Board of Control, • on a sample basis that correct tuition and fees amounts were billed to individual student accounts, • projected tuition and fees revenue based on enrollment data and tuition and fees rates reconciled with tuition and fees revenue in the financial records, and if • on a sample basis that refunds from student accounts were accurate and appropriate. Overall Conclusion Based upon the samples tested and upon our test of tuition and fees revenue as a whole, it is our opinion that tuition and fees were administered according to University policies and that tuition and fees revenue was fairly stated in the financial records. The results of our audit procedures indicated that, overall, • students were charged tuition and fees using rates approved by the Board of Control, • on a sample basis, correct tuition and fees amounts were billed to individual student accounts, • projected tuition and fees revenue based on enrollment data and tuition and fees rates reconciled with tuition and fees revenue in the financial records, and • on a sample basis, refunds from student accounts were accurate and appropriate. Our report has no additional observations.

July 23, 2008 Mrs. Romielyn Romano- Adviser, Political Science Society Allan Quinanola- President, Political Science Society An Informal Trouble Report on Political Science Society’s Executive Officers

ISSUE: The elected secretary of the Political Science society, Angie Ampong had been absent for several meetings. She also failed to pass the minutes of some previous meetings. I called for a General Assembly last July 3, 2008 for the irregularities happened within the Executive Commission. The Representative of the 4th year level, Valentino Bacalso, called the presence of the secretary. Unfortunately, the secretary wasn’t around. The body decided to impeach the secretary on the ground of gross negligence of duty, as provided in the Political Science Society Constitution. Two days after the assembly, I called for emergency meeting, as requested by Ms. Ampong. However, a heated argument happened during the meeting between Ampong and Bacalso regarding with the issue. Ms. Ampong said during the meeting that somebody texted her and asked her to resign and accused the 4th years for making such text message. ACTION TAKEN: As the president of the society, I called the attention of the 4th years. I created a committee to investigate the issue with regards to Ms. Ampong’s issue. The committee decided to give Ampong another chance. The committee also gave Ampong a condition in which failure to do her duty will cause her to be impeached as the secretary of the society. RECOMMENDATION: The society should call for a meeting in which the amendments of the constitution will be discussed. I found out that the constitution has not provided clear details with regards to impeachment. The constitution must also be reviewed. I also recommend to provide the secretary an assistant to help him/her in disposing her duty. The 4th years must be compelled to pay the funds not later than the 1st week of August, as a punishment of sending such text message to Ms. Ampong.
