Please complete this form and return it to our Office on or before JULY 31, 2009. A late fee will be imposed for renewals after AUGUST 15, 2009.

INSTRUCTIONS TO RENEW: 1. FILL OUT APPLICATION FOR RENEWAL LICENSE. Enclose the appropriate fee and updated rabies information, then forward this RENEWAL form to the following address: Ewing Township Municipal Complex Attn: Clerk’s Office 2 Jake Garzio Drive Ewing, New Jersey 08628

* NEW JERSEY STATE LAW REQUIRES THAT YOUR DOG HAVE RABIES PROTECTION THROUGHOUT THE ENTIRE LICENSE YEAR . To ensure that this protection is provided, the Township must get a copy of that proof for their records at the time you RENEW or when you apply for the first time, if you have a NEW DOG. The rabies shot certificate must NOT EXPIRE before APRIL 1, 2010 SO READ IT CAREFULLY. It must also have been signed by a LICENSED veterinarian. ** A Rabies Shot Certificate that expires before APRIL 1, 2010 will require that your dog get a booster shot extending immunization protection for 1 or 3 years. You may contact your personal veterinarian to arrange for this or you may attend one of the Annual Free Rabies Inoculation Clinics sponsored by the Ewing Health Department (883-2900 ext. 7619). Spayed or Neutered - Send in Spay or Neuter ***Documentation obtained from your Veterinarian with RENEWAL form and appropriate payment (Unless already on file). *** All documentation sent as proof (Rabies Certificate, Spaying or Neutering Certificate) will be returned.

By N.J. State Law, no dog license can be issued if the rabies certificate expires prior to April 1, 2010.

You may renew your dog license by mail : Enclose this application when completed, a check made out to the Township of Ewing, and any other required documentation in an envelope and send it to the address noted above. You may also renew your license in person at the Office of the Municipal Clerk between the hours of 8:30 A.M. to 4:00 P.M. on weekdays.

If you no longer own a dog, please notify the Clerk’s Office at 538-7609.
