Tetanus Prophylaxis

Unknown or < 3 doses Clean and Minor Wounds

Only: Toxoid (Start the primary series of Toxoid)

All other wounds (prone)

Give both: Toxoid & IG (Start the primary series of Toxoid)

≥ 3 doses Don’t have to give anything (Unless last dose > 10 years-> give just Toxoid, extra single dose as booster) Don’t have to give anything (Unless last dose > 5 years-> give just Toxoid, extra single dose as booster)

Active : Tetanus Toxoid Passive: ATS (bovine & equine), Human Immunoglobuline Dosage :  Toxoid dose 0.5 mL, whatever the age  immune globulin dosing : o <5-years-old receives 75U o 5-10-years-old receives 125U o >10-years-old receives 250U Application: Toxoid and immunoglobulin should be given intramuscularly with two different syringes in two different locations Primary series of Toxoid:  1st dose now  2nd dose in 1 month  3rd dose in 12 month Cara mengingat: - Pertimbangannya dari sisi Imunisasi-nya dulu, bukan dari sisi bersih atau kotor lukanya. - Pertanyaan yg diajukan :  Apakah pemberian Imunisasi lengkap ? (yg dimaksud lengkap : 3x  Primary series)  Tidak tahu / Tidak lengkap (< 3): maka mulailah dari nol /awal, berikan Primary series (tidak melihat apakah lukanya bersih ataupun kotor)  Ya, Lengkap (3x)  maka tidak perlu diberikan dari awal (Primary Series), cukup hanya Booster saja (1x). Kapan kita berikan? Baru lihat lukanya, Kotor atau Bersih?  Luka yg bersih mempunyai toleransi yg lebih besar (10 tahun) sedangkan Luka yg Kotor mempunyai toleransi yg lebih kecil (5 tahun). -

Jadi pada prinsipnya : baik pada luka Bersih maupun Kotor tidak perlu diberikan APAPUN, bahkan yang Passive (seperti Immunoglobulin) BILA Riwayat Imunisasi (TT) jelas (3x) dan masih di bawah 5 tahun yg lalu diberikannya. Pemberian yang Passive hanya bila Luka Kotor + Riwayat imunisasi tidak jelas
