Mechanism of action and indications Inhibit an enzyme, 3-hydroxy-3 methylglutarylcoenzyme A reductase, which is responsible for catalyzing an early step in the synthesis of cholesterol.
Nursing Implications (what to focus on) Contraindications/warnings/interactions Hypersensitivity, acute hypotension, major surgery, trauma, severe metabolic disturbances, endocrine or electrolyte disorders. Common side effects Dizziness, headache, insomnia, weakness, rhinitis, bronchitis, Abdominal cramps, constipation, diarrhea, flatus, heartburn, altered taste, rashes, pruritus.

Nursing Process- Assessment (Pre-administration assessment) Obtain dietary history, ophthalmic exams are recommended before and yearly during therapy.
Lab value alterations caused by medicine Evaluate serum cholesterol and triglyceride levels before initiating after 4-6 wk of therapy, and periodically thereafter. If muscle tenderness develops monitor CPK levels. Be sure to teach the patient the following about this medication Take medication as directed & do not skip doses, or double up on missed doses. Avoid drinking more than 1 qt grapefruit juice during therapy. This medication should be used in conjunction with diet restrictions, exercise, and smoking cessation. Photosensitivity (rare).
