STUDENT VIDEO RELEASE FORM

Dear Parent/Guardian: The students in my Biology classes are often involved in small group projects, lab work, and class presentations. When the opportunity presents itself, I like to videotape students at work in a typical classroom or lab during these activities or just take a few photos with my digital camera. The student’s performance will not be evaluated in connection with the videotape and names will not be used during any taping. This is not a daily or weekly occurrence, but more spontaneous. When I see something interesting taking place or a nice presentation being made, it is fun to take a photo of the students who are involved. I have enjoyed giving these photos to some of the graduating seniors a few years later. These photos are not for the internet! This note is asking permission for your child’s participation, when/if student images might appear with a class group or lab group at work in a Biology class conducted by Mrs. McKim at Star Valley High School. Please check your decision below. Thank you.
