TROOP 781 ACTIVITY PARTICIPATION AUTHORIZATION EMERGENCY INFORMATION (for parents to keep) Activity: Troop 781Orienteering Day Leader in Charge: Brian Abbey Fee: Park entry fee per car $6.00 Activity Location: Maybury State Park (Enter off 8 Mile Rd, just West of Beck Rd and go to the Walnut & Hickory picnic shelter area nearest the flag pole upon entering the park) Date: Sunday April 19 at 1PM-4PM Emergency # 734-620-4941 Special Instructions: Dress for the weather. Bring water, compass, raingear, insect repellent, sun screen and Scout Handbook. We will be walking through the fields, wear appropriate footwear. First Class Orienteering will be lead by Mr. Abbey. We will coordinate with parents with regard to setting up arrangements for your scout to return home.

I further state that my son or daughter is in good physical health so far as I know, and that he/she has permission to engage in all prescribed activities, except as noted by me. In the event that I cannot be reached in an emergency, I hereby give permission to the physician, selected by the adult leader in charge, to hospitalize, secure proper anesthesia, or to order injection or surgery for my son or daughter.
