Anticoagulant-induced prothrombin deficiency; prophylaxis & therapy of hemorrhagic disease of the newborn & it may also be given to the mother before delivery; hypoprothrombinemia due to oral antibacterial therapy; hypoprothrombinemia

Anaphylaxis, dyspnea, cyanosis, pain, swelling, phlebitis at the Inj site, diaphoresis, dizziness, hypotension (rare), allergic reactions after SC and IM inj.

MECHANISM OF ACTION Phytomenadione promotes hepatic synthesis of clotting factors. It is a naturally occurring compound that is used to prevent and treat haemorrhages related to vitamin K deficiency.

PREGNANCY RISK CATEGORY vitamin K used medicinally in adults (as for blood clotting disorders) is considered a class C drug by the FDA, meaning it is not certain if it is safe for pregnant women and it is not known if it is transferred through breast milk.

INDICATIONS/ USES

Severe anaphylactoid reactions, Diaphoresis, hypersensitivity reactions, Dyspnea, abnormal tastes, Hyperbilirubinemia, Scleroderma-like lesions, Cyanosis, flushing, or hypotension EFFECT ON LACTATION Caution is recommended; benefit to mother should outweigh risk to the infant.

Document the giving of the medication to newborn to prevent an accidental doubling of the dose. Protect drug from light. Give vitamin K before circumcision procedure. May be taken with or without food. Temporary resistance to prothrombin-depressing anticoagulants may result especially when larger doses are used.

INTRAADMINISTRATION Apply pressure to the injection site to prevent further bleeding

POSTADMINISTRATION Observe for bleeding (usually occurs on second or third day). Bleeding may be seen as generalized ecchymoses or bleeding from umbilical cord, circumcision site, nose or gastrointestinal tract. Observe for jaundice and kernicterus, especially in preterm infants. Observe for signs of local inflammation

DRUG CLASSIFICATION Enzymatic mineral and Iron preparation.

DOSAGE/ ROUTE/ FREQUENCY

INDICATIONS/ USES

Tablets – sulfate, 324, 325mg; syrup – fumarate 150mg; elixir – sulfate, 220mg/5mL, drops – sulfate, 75mg/0,6mL, 125mg/mL; tablet, chewable – fumarate, 100mg; suspension – fumarate, 100mg/5mL

An essential mineral found in hemoglobin, myoglobin, and many enzymes. Enters the blood- stream and is transported to the organs of the reticuloendothelial system (liver, spleen, bone marrow), where it is separated out and becomes part of iron stores. Therapeutic Effects: Prevention/treatment of iron deficiency. PREGNANCY RISK CATEGORY Category: A. This supplement should only be used during pregnancy if it is clearly needed.

MECHANISM OF ACTION Ferrous Sulfate is an essential component in the formation of hemoglobin, myoglobin and enzymes. It is necessary for effective erythropoiesis and transport or utilization of oxygen.

SIDE EFFECTS

ADVERSE EFFECTS

GI: GI upset, anorexia, nausea, vomiting, constipation, diarrhea, dark stools, temporary staining of teeth (liquid preparations)

EFFECT ON LACTATION Iron in breast milk is very bioavailable, iron supplementation of the mother does not change this situation. Iron content of breast milk is not affected by the mother's nutritional status.

Give between meals with water but may give with meals if gastrointestinal discomfort occurs. Transient staining of mucous membranes and teeth will occur with liquid iron preparation. To avoid, place liquid on the back of the tongue with dropper or use straw. Assess nutritional status and dietary history to determine possible cause of anemia and need for patient teaching. check the doctor’s order assess for allergy to any ingredient, sulfite; hemochromatosis, hemosiderosis, hemolytic anemias assess for skin lesions, color; gums, teeth (color); bowel sounds monitor blood studies confirm that client does have iron deficiency anemia.

> verify patient’s identity > administer the right drug in the right dose and route at the right time > do not crush, chew or cut tablets and capsules > give drug with meals (avoiding milk, eggs, coffee, and tea) > administer liquid preparations in water or juice to mask the taste and prevent staining of teeth; have patient drink solution with a straw > do not take this drug with antacids nor tetracyclines unless prescribed

>Assess patient for signs and symptoms of anaphylaxis (rash, pruritus, laryngeal edema, wheezing). Notify physician immediately if these occur. Keep epinephrine and resuscitation equipment close by in the event of an anaphylactic reaction. Assess bowel function for constipation or diarrhea.
