Subjective: Hindi ko alam kung pano ang tamang gagawin sa sakit ko. Objective: (+) generalized edema Petechial rashes and foot ulceration (+) ascites Platelet count of 79 x10 L Analysis: Altered health maintenance related to deficient knowledge of disease process and its treatment

The patient has severe alcoholic cirrhosis and poorly controlled diabetes. Knowledge of the client of his condition greatly affect his compliance with medication and treatment. Health maintenance is greatly affected by health teachings.it helps the client and persuade them to comply with treatment. Altered health maintenance is greatly seen in the client because of the evidence of complications.

Dependent: Establish rapport with the client

Instruct client to avoid possible infection and contamination of wounds through proper handwashing, wound care and diet. Explain the disease [process to the client and the possible treatment and medication Inform client that proper compliance and maintenance will help reduce complication.

Cooperation is easily given by the client when trust and rapport is established Proper compliance reduces risk of cmplications

Health education grealy help client to comply with treatment It reduces possible complications

Dependent: Provide wound dressing as ordered

Analysis: Proper health maintenance helps reduce complication in the case of my patient whose condition is irreversible.in such cases nurse function is focused on intellectual and knowledge enhancement of client regarding condtion .

In this case of which client have irreversible disease ; liver cirrhosis and type 11 DM , the objective of care are : 1. To restore and maintain the full potential of the body metabolic function and eliminate or reduce the possible complication that may occur. 2. To impart knowledge and give health teaching to the client and family regarding client case and its treatment to enhance client compliance with medication and treatment.
