1. Assess client’s history and related medical/surgical conditions. 2. Inspect client’s environment for safety hazards. 3. Perform hand hygiene and prepare needed equipment and supplies. 4. Have airway, suction equipment, clean gloves, and pillows available in room. 5. Position client safely if seizure begins. Guide the sitting or standing client to the floor. Cradle client’s head in lap or place pillow beneath it. Clear area of furniture. Lower bed and raise side rails (padded) for client in bed. 6. Provide privacy. 7. Turn client on side, if possible, with head flexed slightly forward. 8. Do not restrain client. Loosen client’s clothing. 9. Do not place anything in client’s mouth. 10. Stay with client. Observe sequence and timing of seizure activity. Note aura, level of consciousness, mobility, incontinence, sleep patterns, or confusion afterward. 11. After seizure, explain occurrence, and offer support. 12. Following seizure, perform hand hygiene and assist client to a position of comfort; place bed in a low position with entitled within reach. 13. For status epilepticus: A. Apply clean gloves. Insert airway when client’s jaw is relaxed between seizure activity. B. Obtain oxygen and suction equipment. Prepare for IV insertion. C. Pad side rails and headboard. 14. Record and report seizure activity and interventions.
