Registration Procedure: Mail this completed form with a check for $50.00 made out to “Volunteer Forces” at least two weeks prior to the start of the course. The $50.00 check must be submitted separately from any other course fee that may apply. The $50.00 check holds your spot in the course and will be returned upon completion of the course. If you are unable to attend, you must notify Volunteer Forces in a timely manner in order to receive a refund. If you fail to notify Volunteer Forces, or just don’t show up, the check will be deposited and used for future trainings and supplies. Submission of the form and check does not guarantee you a spot in the course if the roster is full when your registration is received. If the roster is full when your registration is received you will automatically be placed on an alternate list pending an opening in the course. You will receive an email and/or mailing confirming your spot in the course or that you have been placed on the alternate list.
