Termination by Institution

Student will be terminated from enrollment fc r: 1. Nonpayment — within one month; 2, If a student is found with drugs or alcohol premise: they will immediately be dismissed. They will not be considered for re-enrollment; 3. If they miss 10 consecutive days student will be terminated; 4. if a student is disorderly and refuses to follow ules, regulations and school policies, a student will be terminated.

CANCELLATION POLICY A full refund will be made to any student who cancels the enrollment agreement or contract within 72 )ours (until midnight of the third day excluding Saturdays, Sundays, and legal holidays) after the enrollment contract is signed by the prospective student. If tuition is collected in advance of entrance, and if, after expiration of the 72 hours cancellation privilege, the student does not enter school, not more than $100.00 shall be retained. Full refund will be made if student is not accepted by the school. REFUND POLICY The school has a fair and equitable refund policy, as prescribed by the U.S. Department of Education the Texas Education Code. The school uses the Texas Education Code policy as its refund policy. Enrollment fees & Registration lees are NON-REFUNDABLE. A. SCHOOL REFUNDS

The portion of the period of enrollment or which the student is charged under this policy is determined by dividing the total number of scheduled hours comprising the total length into the number of scheduled hours on roll Ds of the last recorded day of attendance by the student. This percentage is then applied to the refund policy according to the following schedule to determine the amount of ur used tuition and fees to be refunded: (a) during the first week or 1/10 of the program, which( ver is less, 90% of the remaining tuition and fees. (b) after the first week or 1/10 of the program, whichever is less, but within the first three weeks or 1/5 of the course which ever is less than 80% of the remaining tuition and fees. (c) after the first three weeks of the program, but within the first quarter 75% of the remaining tuition and fees. (d) during the second quarter of thr program, 50% of the remaining tuition and fees. (e) during the third quarter of the program 10% of the remaining tuition and fees. (f) during the fourth quarter of the program the student is obligated for the full tuition and fees. For residence program more than 12 months in length the refund shall be applied for each 12 months or part thereof separately.. B, For refund purposes, the effective date of termination shall be the earliest of the following: ( 1) the last day of attendance, if terminated by the school (2) the date of receipt of written notice from the student or {10) ten days after the last date of attendance. The refund computation shall be based on schedu ed hours of class attended through the last day of attendance. Leave of absences, suspensions, school holidays and days when classes are not offered shall not be counted as pay t of the elapsed time for purposes of calculation a student's refund. A full refund of all tuition and fees will be made if it is established that the student's enrollment was procured as a result of any material misrepresentation in advertising, promotional materials of the school or representations by the owner or representatives of the school. All refun is will be consummated in accordance with state regulations not to exceed 60 days. All refunds are made within 30 days from the date the institution has determined that the student is no longer in attendance and no later than 60 days from the actual last day of the student's attendance. Graduation Requirements

Salon & Spa Institute will educate its student: to complete the following essential function for the programs in which they are enrolled. In order to meet the graduation requirements of Salon & Spa Institute and be awarded a certificate of completion, students must complete all hourly requirements, all testing requirements witt a 70% or better, and must meet the financial obligations. Employment Assistance

Although we cannot guarantee employment, upon graduation, we will assist you in any way possible to seek and obtain employment. In fact, we assist you in job placement prior to graduation through different instructional techniques, including résumé writing, interviewing techniques through role-playing. Salon owners and managers also come to the school to conduct mock interviews. We go on field trips, job fairs and hair shows. We provide you with guidance on professional appearance through example. We also post any and all job opportunities in the classrooms and in the break area. Acknowledgement

OUR MISSION: At Salon Et Spa Institute we aspire to inspire. Whether you're interested in a career in Cosmetology, Esthetics, Nail Technology, or nstructor Training, our educators strive to develop the particular interests in each and every on of our students so that they can successfully pass the licensure exam and obtain gainful employment in their field of study. Students here receive extensive hands-on training helping them err power their individual strengths.

ADMISSION REQUIREMENTS

To be eligible to attend Salon a Spa Institut( you must: • be at least 16 years of age • have a valid state issued ID/Driver's license or other government form of identification • pay the required application fee of $50 00 (non-refundable) • pay the required registration fee of $100.00 • have not committed an act that constitu tes a ground for denial of the license • have done one of the following: (a) Co mp le te d a 1 0 t h g r a d e ed u ca i o n o r i t s eq u i v a l en t (b) Have a High School Diploma or GED (c) Pass the Ability to Benefit Test administered by the school, if neither of the above are completed. We use either the Wonderlic Basic Skills Test ( English) or the Wonderlic Scholastic Level Exam (Spanish). • For the 750 Instructor Training courses, you must have a high school diploma/GED and a Cosmetology license. For 250 course, in addition to providing a high school diploma or GED, you must also have two years worts of experience. Transfer Students We will accept students wishing to transfer into Salon a Spa Institute. However, the same admission requirements apply. We will need to obtain proof of hours completed at previous institution and student will be charged at an hourly rate based on how many hours are needed.

APPLICATIONS Each cosmetology student must complete practical applications of the curriculum according to the school's published on minimum practical applications or by the following schedule, whichever is greater. A. Client Protection: 600 applications B. Hairdressing: 600 applications Arranging, cutting, dressing, shampooing, curling, Pressing and finger waving C. Sanitation 500 applications D. Haircoloring: 100 applications Temporary, semi-permanent, Permanent, bleaching And dimensional coloring, color mixing E. SCHOOL FACILITIES Our school is located in a strip plaza on t usy Ruben Torres Boulevard. We have a 5000 square foot building that is beautifully decorated with a warm and inviting atmosphere. We feature 16 hairstyling stations, 8 of which have been made by hand made from solid ash wood and stained to a glossy finish. The spa area is equipped with 3 luxurious pedicure stations built into beautiful cab netry with cushions and pillows to relax and get comfortable. Our pipe less pedicure bowls are soothing and completely sterile. Our manicure room contains 2 manicure stations and is ventilated to promote a clean, relaxing environment. Individual rooms for facials and massages are available to ensure privacy. We also have a reception area, offices, classroom space and restroom facilities.

GRADING SYSTEM Students are graded in theory and practical areas, white clinic work noted towards the required practical applications. Students must pass all tests with a 70% or it must be retaken. Our grading scale is as follows: A

Each student has the right to gain access to her/his cumulative record by appointment under the supervisior of an authorized staff member. Any information pertaining to a student's cumulative record will be released to a third party only up on written instr uction for each request. All of the above rights apply to the parent/guardian if the student is a minor. School employees, schools to which a student is transferring, certain government officials, parties connected to finaicial aid, organizations doing studies for a school, accrediting agency, individuals with a court order or subpoena, persons who need to know for health or safety emergencies and state and local authorities to whom disclosure is required by !tate laws. It is possible to disclose, without consent, "directory information" such as student's name, address and phone number of student, date and place of birth, honors and awards and dates of attendance. However, the school must tell parents of minor students of the information that is designated as directory information and provide a reasonable amount of time to allow the parent of eligible student to request that the school not disclose that information about them. 8

The administrative and instructional staff members at Salon Et Spa Institute are available to help you in any way possible as you rake your way through your course of study. if you are having difficulty fulfilling your academic, attendance or financial obligations, we encourage you to come and speak with any of us so we can design a plan to help you through any challenges you are facing. We also have a list of outside referral agencies available to you in the event the problem(s) you are facing is/are outside our level of expertise. This list is posted in the break area as well if you would prefer seeking out help anonymously. Please know that and thing you tell us will remain confidential.

ATTENDANCE POLICY The student, and or parent, guardian if party to this agreement, agrees that, while in attendance at the school, he/she will be subject to its rules and regulations for the enforcement of proper discipline, and will be studios and punctual and will not, except for urgent reasons absent himself/herself from school. Failure to comply with the rules and/or unsatisfactory scholastic progress is grounds for termination or interruption by the school. Students will be terminated when they are absent for 10 consecutive school days or in excess of 20% of the total clock hours in the course, whichever occurs first.

TARDINESS

1. All students are expected to arrive on time to the facility. On time means, when you have a class theory first thing in the morning or upon your arrival you must arrive at least 10 minutes prior to your theory class, set up and be ready for your day. If you arrive after this time, you will not be able to clock in until after theory concludes. 2. If you are running late, you must notiFy the school so your instructor and any client of yours are notified of your tardiness. If the client wishes he/she will be assigned to another student to accommodate the r time.

EXCUSED VS UNEXCUSED ABSENCES

Every student gets extra time worked into their schedule so we do not have an excused vs. unexcused absence policy in the traditional sense.

MAKE-UP WORK If you miss a test, typically it can be made up on the following Monday; however, you will need to make arrangements with your instructor.

CONDUCT

1. Discussion of Salon/Spa practices, speaking ill of other students and clients is not allowed. Foul or abusive language is forbidden anywhere within this facility. 2. Professionalism is to be strived at all times toward all clients, fellow peers and staff. You are also a role model when not working when you become associated with this establishment. You represent our facility and our beliefs.

OTHER RULES AND REGULATIONS

a. If you will not be attending class for the day, you must notify the school immediately and speak with a school official. Please leave a message with the receptionist. This way your instructor can be notified of your absence and any scheduled appointment can be rescheduled with another stuck nt. b. All students are to eat, snack and drir k in the designated areas of the school (break room). Never at any time is anyone allowed to be eating anything, smoking in front of clients, or hanging out in areas where clients are present. Keep in mind that clients and mirrors are everywhere anc you choice of words could be heard. c. No type of intoxicant is to be consume on premises during school hours and school grounds. Arrival to school under the Influence of any type of chemical stimulant or depressant will be grounds for immedi a te dismissal and/or termination of contract.

d. REMEMBER: • The Client is the boss. • Maintain a constant vigil of your surroundings and your work area. Keep your work area tidy and clean at all times. Sanitation practices are a must at all times. • In a true and real salon spa environment you will be encountered with different work scenarios each time. We encourage all of our students to begin practicing their retail skills, client retention and professional ethics at all times. • Refusal of a client is grounds for immediate dismissal for the day. • Refusing a client in a salon spa environment encourages bad habits and decreases your opportunities for potential it come. • Your style is the Key to Effective Service. You must convey to your customer that you are there for them and them alone. Make your guests feel important. Never ignore or react as though you e not willing to go that extra mile or two for them. Listen to what you client wants and needs, then suggest/recommend but don't insist. •

Leave your problems, bad days, and bad attitudes outside the school doors. This is to be a friendly and relaxing atmosphere for both you and your clients.

SCHOOL SANITATION AND DUTIES POLICY: Salon Et Spa Institute strives to be a c can facility. We do our best to maintain our facility as the first day of business every day. With your help we can continue to provide a clean and sanitary establishment in Which both students and clients can enjoy. Every day you will be assigned a cleaning duty.
