Safety Talk The incident happened in one of the Refinery in Feb’2001 during tank construction
A 65000 M3 Storage tank was under construction by hydraulic jack system.
•
Plate rolling machine was installed at site. The plates were being rolled and were stacked by the side of the rolling site
•
The stacking of rolled plates was done side by side with one of the curved edge as a base.
•
At the time of the incident, seven plates were stacked side by side and were waiting for shifting to grit blasting shed.

Safety Talk •Sudden strong westerly wind started blowing. Being it a construction site the area became dusty and storm like situation prevailed at site •The workers run away from the site in strong wind. •In this situation ,the supervisor also passed through the side of the above said stacked rolled plates. •At this point of time , due to heavy wind all the plates fell down and the supervisor got trapped under the plates, and he was injured severely .
Safety Talk •The plates were removed by hydra and the man was removed from below and taken to Hospital, where he died Recommendations/System Improvements : The rolled plates must be kept in the pipe saddles supports fabricated for this purpose. Pipe supports must be provided on the both sides of the series of stacked plates. Job safety analysis & Daily Tool box talk.
