For RSNA 2009, we’ve secured more than 100,000 room nights at 78 downtown hotels offering the lowest rates in the city. Review the hotel profiles listed on pages 46-47, then follow the registration form instructions on page 50. In the event your hotel selections are no longer available, indicate which is most important: • Cost of your room • Location of your hotel • Other (make note of your preference)

Changes made after November 6 and/or up to 7 days prior to date of arrival that result in a reduction of total room nights will incur a $50 fee charged by the hotel. Changes in arrival and departure dates made less than 1 week prior to arrival that result in a reduction of nights stayed may be considered cancellations by the hotel and may result in forfeiture of partial or all deposits paid. Early departure results in the hotel retaining the fee for a 1-night stay plus tax or the last night’s room deposit of $150, whichever is less. Failure to check in at the hotel on your scheduled arrival day will result in forfeiture of your reservation and hotel deposit. The hotel will accommodate you on a space-available basis.

New: Travel Approval Required for Visa Waiver Countries Visa Waiver Program (VWP) countries are now required to obtain approval through the Electronic System for Travel Authorization (ESTA) prior to traveling to the United States. This requirement applies to all eligible citizens or nationals traveling under the VWP. ESTA is a Web-based system that determines the preliminary eligibility of visitors to travel under the VWP prior to boarding a carrier to the United States. It collects the same information as the paper I-94W form that VWP travelers currently fill out en route to the United States. Travelers without an approved ESTA are advised, however, that they may be denied boarding, experience delayed processing, or be denied admission at a U.S. port of entry. Visitors may apply for travel authorization via the secure ESTA Web site at https://esta.cbp.dhs.gov.

U.S.-VISIT U.S.-VISIT currently applies to all visitors (with limited exemptions) entering the United States. This includes visitors traveling

under the Visa Waiver Program. Visitors are photographed and fingerprinted upon arrival. For more information go to www.dhs.gov/us-visit.

Western Hemisphere Travel Initiative (WHTI) The Western Hemisphere Travel Initiative (WHTI) requires all persons traveling by air between the United States and Canada, Mexico, Bermuda, and the Caribbean region to present a passport or other valid travel document to enter or re-enter the United States. For more information, go to travel. state.gov/travel/cbpmc/cbpmc_2223.html.

International Airfares The Star Alliance™ is a network of participating member airlines. For more information about Star Alliance Conventions Plus airfare discounts and participating airlines, visit staralliance.com/conventionsplus or call any one of the member airlines and use Event Code UA11S09.
