To whom it may concern, I am student at Milligan College working on my thesis and request permission to conduct a survey. The study will try to determine if there is a particular reason that students dislike mathematics. With the current focus on math and science, student’s negative attitude and the reasons for that attitude may give us a clue into what interventions may be possible. The survey will be given to all eighth grade math students who have given informed consent during their math class and will last about 10 minutes. The surveys will be completely confidential. No names or identifying information will be collected and the school will not be mentioned by name. There are no known risks to the students. There are no financial consequences to the school system. The students will be free to ask questions and participation will be voluntary. The student may decline to participate at any point. A summary of the findings will be made available to the school for any interested party. Please indicate your consent for this request by signing below. Thank you for your consideration.
