Risk Awareness

Risk Awareness Agreement The undersigned does hereby acknowledge that their artwork is not covered by insurance while it is on view in the Student Art Gallery or the Furlong Art Gallery or in any area on the campus unless otherwise indicated. The undersigned student does hereby acknowledge that some risk is involved in exhibiting their artwork on this campus and that the Furlong Art Gallery suggests they may consider providing their own insurance coverage. Neither the University of Wisconsin System, nor any individual university in the UWSystem or department thereof, including the Furlong Art Gallery, provides insurance for student artworks. The undersigned further acknowledges that neither the Student Gallery, the Furlong Art Gallery, nor the Department of Art and Design carries any form of medical or accident insurance to aid the student in the case of an accident in the Student Gallery or during the preparation of artwork for presentation in the Student Gallery. The undersigned acknowledges that some risk may be involved during their preparation of their exhibition, therefore they may want to ensure their personal insurance plan covers these activities. PART A I hereby hold harmless the Board of Regents of the University of Wisconsin System and its officers, employees, and agents from any liability which does not arise out of negligent acts or omissions of an University employee, officer, or agents from any liability for damage to or loss of personal property. PART B I understand that the UNIVERSITY OF WISCONSIN-STOUT does not carry any medical or accident insurance for me. If I should become injured, I will be on my own and liable for my own bills should any be incurred.
