[SP579] EVALUATION THE COMPLICATIONS OF ARTERIOVENOUS FISTULA IN DIALYSIS PATIENTS Afsoon Fazelzadeh, Alireza Mehdizadeh, Heshmatollah Salahi, Ghanbar Ali Rais-Jalali, Jamshid Roozbeh, Seyed Ali Malek-Hosseini. Southern Iran Transplant Center, Nemazee Hospital, Shiraz, Fars, Iran. INTRODUCTION AND AIMS: Repeated access to the circulation is essential for performing adequate maintenance hemodialysis(HD). Dysfunction of these fistulas are the most common reason for a 2nd intervention and recurrent hospitalization. The aim of this study was to evaluate the complicated HD fistulas and find the impact of age, site of AV fistulas (proximal or distal), side (left or right) and history of previous vascular access on incidence of complications. METHODS: We studied 273 patients to have any clinical complications that had been developed since beginning of the use of current accesses by history taking and physical examination in every dialysis session. we performed further investigations including doppler ultrasound or spiral CT, to confirm the clinical diagnosis. RESULTS: 40% of our patients had diabetes mellitus as the cause of ESRD. Almost half (49%) the patients dialyzed through an AVF and 13% with a catheter. 184 cases who were 67.6% of under study population had been complicated .Of 145 cases that had elbow AVFs, 103 cases (71%) had complications and of 128 cases with wrist AVFs ,80 cases(62.5%) had complications and there were 115 (62.5%) complicated cases between 185 patients with left AVFs and 69 (78%) complicated cases between 88 patients with right AVFs and the rate of AVF complications increased with age.The one year survival rate was 94%. We didnt find any significant difference between diabetes mellitus or hypertension as the underlying cause of renal failure and AVF complications. Mean cholesterol plasma level did not differ significantly between the patients with and without AVF complications. CONCLUSIONS: In conclusion we found that rate of complications increased with age (P value<0.05). Mean hematocrit levels were not significantly different between the two groups.On the other hand, mean EPO weekly dose was significantly higher in the groups of patients with AVF complications Our results showed that the frequency of complications is higher in patients with elbow and right side AVFs and also patients with history of previous failed shunt but no valuable statistical relationship was found between these variables (P value>0.05).
