Acute pyelonephritis • Grossly – Normal or Enlarged, swollen kidney – C/S – Abscesses -small yellow-white with hemorrhagic rim, Mainly in cortex
• Microscopy – Acute inflammation – Interstitium and destruction of tubules – Glomeruli and blood vessels – Spared – Sheets or focal abscesses of neutrophils
Chronic pyelonephritis • Gross – Small, asymmetrically, contracted kidney – <100 g – Surface – Irregularly scarred, and cortical depressions – Dilated pelvis, blunted calyces
• Microscopy – Interstitium – Chronic inflammation, lympho, plasma, macro and fibrosis – Tubules – Atophy, dilation, thyroidisation – Pelvicalyceal system – Inflammation, scarring – Glomeruli – Peri-glomerular fibrosis, hyalinisation
