Waiver & Release Form Because physical exercise can be strenuous and subject to risk of serious injury, we urge you to obtain a physical examination from a doctor before using any exercise equipment or participating in any exercise activity. I, the undersigned, being aware of my own health and physical condition, and having knowledge that my participation in any exercise program may be injurious to my health, am voluntarily participating in a physical activity, and do so at my own risk. Having such knowledge, I hereby acknowledge and release Total Training by Tammy, Tammy Niemann, and any representatives, agents, and successors from liability for accidental injury or illness, which I may incur as a result of participating in the said physical activity. I hereby assume all risks connected therewith and consent to participate in said program. I agree to disclose any physical limitations, disabilities, ailments, or impairments, which may affect my ability to participate in said fitness program. I choose to participate of my own free will. In consideration of the permission to participate extended to me and for the services furnished to me; I do hereby for myself, my heirs, spouse, children, unborn children, personal representatives, and agents release and forever discharge any and all claims, demands, actions or lawsuits on account of my injury or death that might occur as a result of negligence on the part of myself or other persons affiliated or not affiliated with this contract.. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from. Physical Activity Readiness Questionnaire Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people. However, some people should check with their doctor before they start becoming much more physically active. If you are planning to become much more physically active than you are now, start by answering the seven questions in the box below. If you are between the ages of 15 and 69, the PAR-Q (Physical Activity Readiness Questionnaire) will tell you if you should check with your doctor before you start. If you are over 69 years of age, and you are not used to being very active, check with your doctor.

Frequently Asked Questions What do I need to bring to boot camp? You will need to bring the following: • One set of 5-8 lb. dumbbells • An exercise / yoga mat • A water bottle • Towel • Completed Registration Packet Since we meet outdoors, we advise dressing in layers (you will get warm), and don’t forget to wear sunscreen, comfortable gym clothes, and athletic shoes. What if I am not very fit? Our Fitness Boot Camp classes and personal training programs are designed to meet the needs of people of all different fitness levels. We will instruct you on how to alter the exercises so that you will be challenged whether you are a beginner or an advanced fitness enthusiast. Will you yell at me? We’ll challenge you, we’ll push you, and we’ll encourage you to try new activities that will test your strength, endurance, and courage. But we won’t yell, intimidate, or get in your face. We don’t need to. The motivation and friendly competition provided by your fellow Boot Campers will be all the motivation and encouragement you need to reach your goals!. What kind of people are enlisted at Boot Camp? Our Boot Camp attracts people of all shapes, sizes, ages and fitness levels! Our Boot Camp classes are great for: 9 to 5ers' who need to release stress, mom's that want to lose baby fat, busy professionals who want to get focused, students who need to get back on the right track, as well as conditioned athletes who want to step it up a notch. Our Fitness Boot Camp is basically for anyone who wants something different and wants RESULTS! What type of exercises will I be doing? Resistance training and cardiovascular exercise such as: Running (short distances), Push-ups, Sprints, Lunges,Jumping Jacks, Jump rope, Mat work, Weight Training, Stability Ball/Medicine Ball training, Stretching, Cone drills, Obstacle courses, and whatever else it takes to distract you from the fact that you're getting a great workout! Every day will be a completely different workout – never predictable – never boring and always FUN! What if I sign up for the two or three day plan and then want to upgrade to the 5-days a week camp? Don’t worry, after starting camp, many people realize how fun and convenient our Outdoor Fitness Boot Camp is and want to sign up for more days. They also realize that they will achieve greater results if they attend more often. In these situations, we will simply pro-rate the additional fee from the time you decide to sign up for more days. We understand that unpredictable situations can arise, so you can make up any missed class within that four week schedule. You can make up the missed class by attending any of our class times and/or attending more than one session in a day. If you notify us that you have a need to miss a class at the time of signing up, you can either make it up as outlined above or we can deduct the pro-rated amount from the purchase of your next four week session. Where do I park for the Outdoor Boot Camp? There is ample free parking surrounding University Community Park, Gabrielino Community Park and Lake Park. Do we still meet for the Outdoor Boot Camp if it rains? Yes, we meet rain or shine so please come dressed appropriately. What if I have an injury? Please let us know immediately so we can advise you on how to alter the exercises in order to prevent further injury.
