Intervention People: Raise Concerns to the management Management: Raise concerns to the management Environment: There should be teamwork and commitment to the task in the team. Process: If relatives will be designated to drain the foley catheter, a proper health teaching should be done. There should be a person making sure that vital signs are monitored

Evaluation Proper drainage of foley catheter is done and there is less incidence of inaccurate
