1. References: Attendance sheet of personnel PM Shift dated January 28, 29, 31 And February 2. In connection with the above references, the undersigned PNCO was directed to explain in writing on why he should not be charged for administrative case for failure to attend DPAR on February 1 2019(Friday). 3. Please be informed that the undersigned PNCO was not able to attend DPAR on February 1 due to I woke up late as a Night Shift Duty. However, the undersigned reported to Lacson cor. Laon Laan for DPAR but unfortunately it was already done and not able to sign the attendance sheet (DPAR). 4. Hence, the undersigned reported on duty at Lacson PCP as Line Beat Patroller from 10:00 PM to 10:00 AM (Night Shift) on January 29, 2019. Attached herewith is the Daily Attendance Report at Lacson PCP as of January 29, 2019. And certification from PCP Commander. 5. In this regard, he prays that his explanation may merit your understanding and that he may be exonerated from any administrative liabilities your office may file against him. Rest assured that the undersigned will utilize the fullest of his capabilities to be more effective in his line of work in order to prevent the same mishap from happening again.
