Phonological Awareness Instructions: There are ten items that will be assessed. A sample is provided for each phonological awareness skill. Items are to be assessed initially in September for K-4 students and in January for K-3 students. You will use the Phonological Awareness Direction/Scoring Sheet to record student responses. Mark an x in the box at the end of the row for each correct response. After the assessment, record the total number correct in the box marked ‘total correct’. Item #1 and item #2 assess rhyming. Say the target word. Tell the child to listen for a word that rhymes with the target word. Say each of the word choices aloud for the child. Ask the child to tell you which word rhymes with [say target word]. The child must orally tell you the rhyming word (not just point). Item #3 and item #4 test the child’s ability to identify words in a sentence by clapping each word. Read the sentence to the child. Then, have the child repeat the sentence by clapping each word. Item #5 and item #6 assess the child’s ability to recognize that 2 or more spoken words begin with the same sound. (alliteration) Say the target word. Tell the child to listen for a word that begins the same as the target word. Say each of the word choices aloud for the child. Ask the child to tell you which word begins with the same sound as [say target word]. The child must orally tell you the correct word (not just point). Item #7 and item #8 test the child’s ability to blend 2 words to form a compound word. Read the two words to the child. (No picture cards will be used.) Tell the child to orally put the words together to make a compound word. (If the two words are: cup and cake, the acceptable response is “cupcake;” not “cup” “cake.”) Item #9 and item #10 assess the child’s ability to segment words into syllables. Say each word for the child. Have the child repeat the word and segment the word. Both K-3 and K-4 students are expected to be able to segment the word correctly.
