Requirements: 1. User friendly controls should be placed. 2. Patient should provide comments in a Text Area control. 3. All the controls should be properly data bind to the feedback table. Checklist: 1. Patient views this page. 2. Patient enters the details and feedback. 3. Patient submits the feedback by clicking Submit button. 4. Data is stored in the feedback table. 5.
