When a specimen has been obtained from the patient it must undergo standardized, validated process from delivery of the specimen to the department until it is ready for diagnosis by the pathologist. These procedures are carried out in different areas of the department. 1. Specimen reception The main work carried out in specimen reception is receipt of specimens from porters. These specimens are delivered along with the specimen request form containing patient and clinical details as shown below. They are both then quality control checked for non conformances, inadequate fixation and for any other special requirements. Any discrepancies with specimen details on pot or request form are checked on the computer system PAS and then a senior staff member if the discrepancy is still unresolved. If the discrepancy is still unresolved then the specimen is sent back to the source. If the specimen is low on formalin or has none in it, then the container is checked to see whether it is leaking due to broken container. If so then a new container is located and topped up with formalin using formalin tap and pump
