PLANNING
Subjective: “Kumakalat ang rashes sa balat ko” as verbalized by the patient Objective” >presence of rashes >frequent scratching >disruption of skin surface
Impaired Skin Integrity r/t alteration in skin appearance as manifested by presence of rashes
At the end of the shift, the patient will be able to demonstrate behaviours/ techniques to prevent skin breakdown
INTERVENTIONS >Recommended keeping nails short. >Kept the area affected clean and dry >Removed wet/ wrinkled linens >Instructed patient not to use tight clothings >Administered triderm as ordered.
RATIONALE >To reduce risk of dermal injury when severe itching is present >To prevent further invasion of microorganism >Moisture potentiates skin breakdown >To prevent skin irritation >To decrease irritable itching
EVALUATION Goal met. The patient was able to demonstrate behaviours/ techniques to prevent skin breakdown.
