AMERICANS FOR SAFE ACCESS ADVOCACY GRANTS PROGRAM Questionnaire (All questions should be answered separately and should be no longer than three  pages total.)   A. What is the objective of the project?  B. What is the project plan?  C. How will you know if you meet your goals?  D. How does this project fulfill ASA's mission?  E. What is the budget of the project?  F. How will you raise any additional funds needed?  G. Applicant's information: brief description of mission, history, accomplishments, and current  activities.  H. Non­profit applicants should list Board officers and directors, with contact information for all.  I. Applicant's financial statements for past three years (if available), and organizational budget for  current fiscal year and/or next fiscal year.   J. Applicants seeking 501(c) (3) funds ­­ which is a plus but not required ­­ should include the IRS  letter of recognition.
