Little Seeds Parent Agreement

Little Seeds Home Daycare Parent/Provider Agreement Bismillah ir rahmaan ir raheem This is not a contract, just merely a written agreement between you the parent and me the childcare provider. This agreement is made by and between Makeydah Amat al Khaliq-Jones, Child Care Provider and _______________________________, Parent/Guardian of __________________________________. The following has been agreed upon between the two parties beginning _____________: I have read and agree to full contents of the Parent’s Handbook. I agree to the weekly rate of $_______, to be paid on Monday at the beginning of weekly care. I agree to pay a deposit/admin fee of $15 to hold a space until ____________. In the event I choose not to enroll my child with this provider this deposit is non-refundable. THIS AGREEMENT AND THE PARENT HANDBOOK WHOLLY STATE THE OBLIGATIONS OF THE PROVIDER; THERE ARE NO OTHER IMPLIED OBLIGATIONS. ANY AMENDMENTS TO THIS AGREEMENT MUST BR IN WRITING AND SIGNED BY BOTH PARTIES.
