H/O bilateral leg pain and Back pain. CT myelography of lumbar spine has been done by injecting dilute water soluble contrast in the lumbar subarachnoid space under LA. Axial images have been taken through the vertebral bodies and discs. A large diffuse disc bulge is seen at L3- L4-L5 causing marked thecal sac compression and bilateral L5 exiting nerve root compression. The disc bulge at L3-L4 has contacted the left L5 traversing nerve root causing grade I compression. Marked reduction in thecal sac areas is noted also at these levels indicating thecal sac stenosis. Other discs look normal. Visualised portions of both SI joints look normal. No destructive bony lesion is noted. Impression: A large diffuse disc bulge is seen at L3-L4-L5. And thecal sac stenosis.
