7th Grade Field Day Information/Permission Sheet May 19th, 2009 Dear Parent or Guardian: The seventh graders will end the 2008/09 school year with a Field Day DATE:
Wednesday, June 10th (Rain date Friday, June 12th)
TIME: LOCATION:
12:45pm till 3:00pm On the grounds of Crestwood
All 7th grade students will participate in Field Day. Students who receive an office referral, that leads to suspension on or after Monday, June 1st will not be permitted to participate in the field day without a parent chaperone. If a parent cannot attend those students will be provided alternate activities. All parents must sign and agree to the terms of the Field Day. We are looking for parent/guardians to help with this event. We need parents to assist in the management of activities and/or with food stands. We also need donations of bottled water. If you are interested in either donating water or being a chaperone, please check the appropriate box and provide a contact number.
